摘 要:目的通过CT血管成像技术对腰动脉的影像解剖进行观测并探讨其临床意义。方法 2012年3月至2012年6月,第三军医大学西南医院放射科行CT腹部血管成像检查的55例,使腹主动脉及腰动脉清晰显示。观测腰动脉起点平面及缺失,腰动脉发出处与腹主动脉形成的上夹角及其内径,腰动脉发出处相邻腰动脉间的距离及腰动脉共干现象。结果大多数腰动脉发自相应的腹主动脉对应椎体平面。共9例(16.4%)腰动脉缺失。内径自上而下逐渐增大,两侧比较差异无统计学意义(P<0.05)。第1、2、3腰动脉发出处和腹主动脉所形成的上夹角为锐角,第4腰动脉为钝角。相邻腰动脉间的距离自上而下逐渐变小。共3例(5.5%)腰动脉发出处有共干现象。结论腰动脉局部解剖变异较大。行腰动脉介入治疗前要了解腰动脉的局部解剖特点,警惕脊髓血供。Objective To observe the imaging of lumbar arteries with computed tomographic ( CT ) angiography and to explore the clinical significance. Methods From March 2012 to June 2012, 55 patients accepted abdomen CT angiography, and both aorta abdominalis and arteriae lumbales were clearly displayed. The starting location and deficiency of lumbar arteries, the angle formed between the starting location of lumbar arteries and the aorta abdominalis and the inner diameter were observed. The distance of adjacent lumbar arteries from the starting location of lumbar arteries were measured. The same starting point shared by adjacent lumbar arteries were detected. Results Most of the lumbar arteries originated from the vertebra/body plane of the corresponding aorta abdominalis. A lack of lumbar arteries was found in 9 cases ( 16.4% ). The inner diameter was gradually increased from top to bottom, and no statistically significant differences were observed between the left and right side ( P〈0.05 ). An acute angle was formed between the 1st, 2nd and 3rd lumbar arteries and the aorta abdominalis respectively, and an obtuse angle was formed between the 4th lumbar artery and the aorta abdominalis. The distance of adjacent lumbar arteries from top to bottom was gradually reduced. The same starting point shared by adjacent lumbar arteries was noticed in 3 cases ( 5.5% ). Conclusions Obvious local anatomical variations can be observed in lumbar arteries. Before the interventional treatment, the local anatomical characteristics of lumbar arteries should be mastered, and the blood supply of the spinal cord cannot be affected.
