高压氧联合甲钴胺治疗2型糖尿病周围神经病电生理疗效观察

摘　　要：目的:探讨高压氧(HBO)联合甲钴胺治疗2型糖尿病周围神经病(DPN)的电生理疗效。方法:DPN患者69例随机分成对照组36例和治疗组33例,对照组给予甲钴胺治疗,治疗组给予甲钴胺治疗的同时给予HBO治疗;治疗前后均行电生理检查。结果:治疗组治疗后正中神经指1及指3、尺神经、腓肠神经、腓总神经及胫神经感觉神经传导速度(SCV)、感觉神经动作电位(SNAP)波幅及运动神经复合肌动作电位(CMAP)波幅均高于治疗前(P<0.05),CMAP潜伏期低于治疗前(P<0.05);对照组治疗后正中神经指1及指3、尺神经SCV均高于治疗前(P<0.05)。治疗后,治疗组正中神经指1及指3、尺神经SCV较对照组明显提高(P<0.05)。结论:HBO联合甲钴胺治疗DPN效果显著。Objective： To investigate the electrophysiological changes after of treatment of hyperbaric oxygen（HBO） combined with mecobalamin in patients with type 2 diabetic peripheral neuropathy（DPN）. Methods： Sixty-nine cases of patients with DPN were randomly divided into control group（n=36） and treatment group（n=33）.Cases in the control group were given mecobalamin treatment, and those in the treatment group were given both mecobalamin and HBO treatment. The electrophysiological examination was performed before and after the treatment. Result： In the treatment group, the median nerve, ulnar nerve, ulnar nerve, sural nerve, common peroneal nerve and tibial nerve sensory nerve conduction velocity（SCV）, sensory nerve action potential（SNAP） amplitude and compound muscle action potential（CMAP） amplitude were higher than those before treatment（P〈0.05）, and CMAP latency was shorter than that before treatment（P〈0.05）. In the control group, the median nerve and ulnar nerve SCV were higher than those before treatment（P〈0.05）. The median nerve and ulnar nerve SCV in the treatment group were higher than those in the control group after treatment（P〈0.05）. Conclusion： HBO combined with Methylcobalamin is effective to treat DPN.
