慢性瘘管型根尖周炎一次性根管治疗效果临床评价

摘　　要：目的探讨一次性根管治疗慢性瘘管型根尖周炎的临床疗效。材料和方法选取2013年1月至2014年9月来我科就诊的265例、306颗瘘管型慢性根尖周炎患牙,随机分为治疗组165颗患牙,行一次性根管治疗术;对照组141颗患牙,行传统多次性根管治疗术;对两组进行疗效对比。结果两组患者临床疗效比较,治疗组共治愈160颗,占总有效率96.97%,对照组共治愈134例,占总有效率95.04%,两组患者术后1周疼痛及6-12个月后治愈总有效率相比差异无统计学意义,p>0.05。结论一次性根管治疗慢性瘘管型根尖周炎的术后疼痛及根尖周愈合率与多次疗程差异无统计学意义,但患者复诊次数少,疗程短,省时及医疗费用低,值得在临床推广使用。Objective To evaluate the clinical effects and reliability of one- visit endodontic therapy for fistula of chronic diseases of periapical.Methods 306 teeth with fistula of chronic periapical periodontitis were investigated. 165 teeth(one- visit group)were treated with one- visit canal obturation, and 141 teeth(multi- visit group) were treated with canal obturation after multi- visit therapy, and the clinical effects of two groups were compared. Results Clinical effects of two groups were not statistically significant. 160 teeth were cured in one- visit group, abont 96.97% of the total; 134 teeth were cured in multi- visit groep, about 95.04% of the total.The pre- and post- operative pain in 1 week of two groups, and the differences of 6- 12 months cure rate had no statistical significance(p >0.05). Conclusions Compared with multi- visit therapy, the post operative pain and6- 12 months cure rate of one- visit endodontic therapy for teeth with fistula of chronic diseases of periapical has no significant difference but its short- course therapy, less return visit and medical expenses are worth clinical promotion.
