玉女煎对实验性犬种植体周围炎的治疗作用

摘　　要：目的:研究中药"玉女煎"对犬种植体周围炎的治疗作用,探讨中药治疗种植体周围炎的疗效。方法:建立犬种植体周围炎模型,随机分为4组,模型对照组、中药组、局部用药组以及合用组。局部用药组与合用组采用派丽奥牙科软膏局部给药,每次每牙50 mg局部牙周注射用药处理,中药处理组与合用组每天均按设计剂量以玉女煎汤药灌胃3次,另外两组每日以等量生理盐水灌胃3次,分别给药6周,给药量依据人和动物间按体表面积折算的等效剂量比率表。每周检测各组临床牙周指标——龈沟探诊出血指数(gingival sulcus bleeding index explore examine,BOP)、牙周探诊深度(periodontal depth probe diagnostics,PPD),并通过组织病理学观察,了解中药"玉女煎"对种植体周围炎的治疗效果。结果:合用组BOP出现了较明显的下降;各给药组PPD随治疗时间的延长均出现了不同程度的下降,其中合用组下降最显著(P<0.05);各治疗组PPD值与模型组比较差异均有显著性意义(P<0.05)。模型对照组标本中聚集大量炎症细胞,扩张血管明显增多、出血现象明显。牙槽骨中存在明显的骨吸收陷窝,骨质吸收严重,牙周袋深在,而单纯中药组与局部用药组炎症细胞浸润有所减轻,合用组炎症细胞数量明显减少,骨吸收陷窝及活性破骨细胞数明显少于对照组,牙槽骨吸收较少,并且有所恢复。结论:中药"玉女煎"方剂合并局部用药对种植体周围炎具有明显疗效。Objective： To study the curative function of Jade Maid Decoction on dogs＇ peri-implant inflammation,and to explore the new methods to treat peri-implant inflammation by using Chinese medicine. Methods： The dogs＇ peri-implant inflammation models were established and then divided into the model control group,the Chinese medicine group,the local administration group,the combination group. The patients in the local administration group and the combination group were given local administration with Periocline dental ointment; Each tooth＇s periodontal was injected into 50 mg each time. The Chinese medicine group and the combination group were treated with administration by gavage with Jade Maid Decoction,3 times a day. The other 2groups were treated with administration by gavage with the same amount of saline. All these groups were treated for 6 weeks. The dosage was based on equivalent dose rate table at the conversion of body surface area between human and animal. Each week each group＇s clinical periodontal indexes were tested,including gingival sulcus bleeding index explore examine（ BOP）,and periodontal depth probe diagnostics（ PPD）; The curative effect of Jade Maid Decoction on peri-implant inflammation was obtained by observing pathology. Results： BOP of the combination group decreased clearly; PPD of each group decreased with different degrees with the prolonged time,especially the decreased degree of the combination group was the most obvious（ P〈0. 05）. The differences in PPD between each treatment group and the model group had significance（ P〈0. 05）. In the model control group,a large number of inflammatory cells appeared in the samples; Dilation of blood vessels increased; And the bleeding phenomenon was more obvious; There was obvious lacuna of bone in the alveolar bone; Bone absorption was serious; Periodontal pockets were deep.
