扩大医疗保险覆盖面有利于经济发展和社会福利改善吗

摘　　要：医疗保险作为社会保障的重要组成部分,普遍认为其可以提升社会福利水平,进而保障并促进经济社会发展,因此不断扩大其覆盖面成为必然。根据动态可计算一般均衡理论建立医疗保险各方主体行为模型,并通过对比医疗保险覆盖面扩大前后的模拟结果,发现经济效率确实有所改善,但也出现了资本积累率下降和劳动力供给的扭曲,经济产出不增反减,社会代际福利的总体水平也有所降低。因此,调整医疗保险扩面举措及相应政策尤为必要,应适度放松计划生育政策,提高加强医疗保险环节间的精算联系,探索名义账户制度和调整医疗保险费率设定,以积极应对其对经济发展和社会福利的负面冲击。Health insurance is regarded as an important part of social security that could raise the general level of social welfare,and thus safeguard and promote economic and social development,thus expanding its coverage is inevitable. This paper,based on the establishment of a dynamic computable general equilibrium theory,characterizes the model of the behavior of the parties,and by comparing simulation results expands the health insurance coverage before and after the discovery. It concludes that economic efficiency has improved,but there has been a decline in the rate of capital accumulation and labor distortion of supply,and economic output has reduced the overall level of the international community on behalf of reduced welfare.Therefore,we believe that extending the coverage of health insurance adjustments and appropriate policy initiatives is particularly necessary,and should be appropriate to relax the family planning policy to improve the actuarial links between the health insurance segments,explore the notional account system and adjust the health insurance rate setting,with a positive response to the negative impact on economic development and social welfare.
