PAX2在Ⅰ型子宫内膜癌及其癌前病变中的表达及意义

摘　　要：目的:探讨PAX2在Ⅰ型子宫内膜癌中的表达,分析其在Ⅰ型子宫内膜癌发病机制中的作用。方法:采用免疫组化法检测61例子宫内膜癌、35例子宫内膜增生、19例子宫内膜不典型增生及17例正常增殖期子宫内膜中PAX2的表达情况。结果:(1)PAX2的表达在不典型增生内膜、Ⅰ型子宫内膜癌与正常增殖期子宫内膜之间;不典型增生内膜、Ⅰ型子宫内膜癌与子宫内膜增生之间;不典型增生内膜、Ⅱ型子宫内膜癌与Ⅰ型子宫内膜癌之间差异均有统计学意义(P<0.05)。而PAX2的表达在正常增殖期子宫内膜、子宫内膜增生、Ⅱ型子宫内膜癌之间两两比较均无差异(P>0.05)。(2)61例子宫内膜癌中,有淋巴结转移的患者其PAX2的表达明显高于无淋巴结转移的患者(P<0.05),差异有统计学意义。而PAX2的表达与患者的年龄、分化程度以及肌层浸润情况均无关(P>0.05)。结论:PAX2基因可能在Ⅰ型子宫内膜癌癌变的机制中扮演了重要的角色,并可以成为鉴别子宫内膜过度增生以及不典型增生的染色指标。Objective： To investigate the expression of PAX2 in endometrial carcinoma and precancer, and analyze the role of PAX2 play in the pathogenesis of type I endometrial carcinoma. Methods： The expression of PAX2 was detected by immunohistochemical staining in 61 cases endometrial carcinoma, 35 cases endometrial hyperplasia,19 cases atypical endometrial hyperplasia and 17 cases of normal proliferative endometrium. Results： （1） Between atypical endometrial hyperplasia,type I endometrial carcinoma and normal proliferative endometrium, atypical endometrial hyperplasia, type I endometfial carcinoma and endometrial hyperplasia, atypical endometfial hyperplasia, type Ⅱ endometrial carcinoma and type I endometrial carcinoma, the positive expression of PAX2 have significant differences （P〉0.05）.However,there were no differences among normal proliferative endometrium, endometrial hyperplasia and type U endometrial carcinoma（P〈0.05 ）.（2） The positive expression of PAX2 in patients with lymph node metastasis was significantly higher than that in patients without lymph node metastasis （P〈0.05）, but there were no associations can be seen between expression of PAX2 with age, histological grade and myometrial infiltration. Conclusion： PAX2 gene may play an important role in the pathogenesis of type I endometrial carcinoma,and which can be as a maker for diagnosis of endometrial hyperplasia and atypical endometrial hyperplasia.
