摘 要:目的研究中、重度牙周炎患者龈沟液中RANKL、OPG的浓度及经牙周基础治疗后龈沟液中RANKL、OPG及其比值的变化,以期为临床提供参考。方法随机选取40例中、重度牙周炎患者为牙周病组进行牙周基础治疗,采集治疗前及治疗后3周龈沟液,测量所含RANKL、OPG浓度,同期检查40例健康者作为对照组。结果 1治疗前牙周病组GCF中RANKL浓度为53.13±l4.43pg/nl,OPG的浓度为69.86±20.98 pg/nl,RANKL/OPG比值为1.57±1.18;对照组GCF中RANKL浓度为28.90±12.57pg/nl,OPG的浓度为109.08±49.35 pg/nl,RANKL/OPG比值为0.33±0.25。三组数值差异均具统计学意义(P<0.01)。2经牙周治疗后牙周病组患者RANKL浓度降为40.56±9.63;OPG浓度提高为88.96±17.24pg/nl;RANKL/OPG比值为0.44±0.32,较治疗前均具有统计学意义。3牙周病组患者治疗前后的AL和PD的变化与GCF中RANKL浓度变化呈正相关关系(相关系数分别为r=0.586,P=0.032;r=0.601,P=0.031);与OPG浓度变化无明显相关性(相关系数分别为r=-0.176,P=0.532;r=-0.187,P=0.519)。结论 1慢性牙周炎患者GCF中RANKL的含量升高,OPG的含量降低,提示牙槽骨的吸收可能与牙周局部RANKL和OPG的比例失调有关。2基础治疗对牙周病控制有一定作用。3可考虑干预OPG/RANKL/RANK通路影响牙槽骨的改建,从而找到更有针对性的牙周炎治疗方法。Objective To investigate the effect of periodontal basic treatment on the osteoprotegerin(OPG)and receptor activator of nuclear factor-KB ligand(RANKL) in the gingival crevicular fluid(GCF) of patients with chronic periodontitis. Methods Forty patients with periodontal were selected as experimental group while 40 health people is regarded as control group. The GCF of 80 subjects were collected before and 3 weeks after basic treatment. The concentration of OPG and RANKL in GCF were detected by enzyme linked immunosorbent assay(ELISA). Results 1The levels of RANKL, OPG and RANK/OPG in GCF of the experimental group were 53.13±l4.43pg/nl, 69.86±20.98pg/nl and 1.57±1.18 in constrast to those of the control group(28.90± 12.57pg/nl, 109.08±49.35pg/nl and 0.33±0.25). All the three sets of data are satistically significant(P0.01). 2The concentration of RANKL in experimental group(40.56±9.63pg/nl) after treatment was lower than that before treatment(53.13±l4.43pg/nl) and there was significant difference between before and after treatment(P0.01). The concentration of OPG in experimental group(88.96±17.24pg/nl) after treatment was higher than that before treatment(69.86±20.98pg/nl) and there was significant difference between before and after treatment(P0.01). The ratio of RANKL/OPG in experimental group(0.44±0.32) after treatment was lower than that before treatment(1.57±1.18)and there was significant difference between before and after treatment(P0.01).3There was a significant positive correlation between the concentrations of RANKL and the change AL, PD before and after treatment(r =0.586, P =0.032; r =0.601, P =0.031). There was no significant correlation between the concentrations of OPG and the change of AL, PD before and after treatmen(r=-0.176, P=0.532; r=-0.187, P=0.519).Conclusion 1 The high level of RANKL and the low level of OPG in GCF of the patients with chronic periodontitis mean a relationship between the imbalance of RANKL and OP
