摘　　要：目的 探讨带血管髂骨瓣重建桡骨远端骨折并软骨缺损的临床疗效.方法 自2013年6月至2014年6月我们对5例AO分型为C型的桡骨远端骨折并软骨缺损患者采用带血管髂骨瓣移植重建关节面.术后随访腕关节掌屈、背伸、桡偏、尺偏的活动范围.结果 术后随访时间为6～12个月,平均9个月.按中华医学会手外科学会上肢功能评定试用标准评定：优2例,良2例,可1例.所有患者均无钉道感染及肌腱损伤等并发症.结论 带血管髂骨瓣移植能重建桡骨远端骨折并软骨缺损,恢复关节面的完整及腕关节功能,是治疗桡骨远端骨折并软骨缺损的有效方法.Objective To investigate the clinical efficacy of the vascularized iliac bone flap reconstruction of distal radius fractures and cartilage defects.Methods From June 2013 to June 2014,five cases of distal radius fractures and cartilage defects in AO classification type C were treated with vascularized iliac bone graft reconstruction of the articular surface.Postoperatively the range of motion of wrist flexion,dorsiflexion,radial deviation,and ulnar deviation were evaluated.Results The time of postoperative follow-up was 6 to 12 months,with an average of 9 months.Based on the upper limb functional evaluation criteria issued by the Hand Surgery Society of the Chinese Medical Association,the results were rated as excellent in 2 cases,good in 2 cases,and fair in 1 case.There were no complications,such as pin tract infection or tendon damage.Conclusion Vascularized iliac bone graft can reconstruct distal radius fractures and cartilage defects,restore the integrity of the articular surface and function of the wrist.It is an effective method for treatment of distal radius fracture with cartilage defects.
