摘  要:目的探讨核医学单光子发射计算机断层摄影(single proton emission computed tomography,SPECT)检查方法对裸鼠肺肿瘤临床靶区确定的应用价值。方法建立稳定表达luciferase A549-luc+肺肿瘤裸小鼠模型,在同一肿瘤模型中分别获得肿瘤荧光图像与SPECT图像,应用Matlab软件计算机平台开展2种图像的对比实验研究。通过对这2种图像的数值计算和比较分析探索肺肿瘤的形态和病理细胞边界分布特征。不同测量方法得到的数据分成荧光组和SPECT组,按照配对资料分析。结果肿瘤荧光图像周长和SPECT图像周长样本为非正态分布,两组差异有统计学意义,z=2.12,P=0.034。肿瘤荧光图像面积和SPECT图像面积样本为正态分布,两组差异有统计学意义,t=165.75,P<0.001。裸小鼠肿瘤荧光图像略小于SPECT图像。图像对比研究发现,SPECT图像边缘放射性强度峰值分布不均匀,而荧光图像边缘在瘤细胞数量较少时没有形成具有可信度的荧光图像。移植瘤模型区域细胞学移行行为表现出复杂的由瘤体中央向边缘过渡而逐步减少的非线性关系。结论与荧光图像比较,SPECT处理的同一肺肿瘤裸小鼠模型病理细胞学图像边界欠清晰,但是SPECT反映的是肿瘤边缘细胞移行中的散在细胞学特征,而荧光图像不具有这些特征,提示核医学图像对裸鼠肺肿瘤临床靶区的确定是一个不容忽视的重要补充。OBJECTIVE To evaluate the role of nuclear medicine SPECT scans for determinating the CTV of lung tumor of nude mice. METHODS A nude mouse model with human A549 cells was established. Tumor fluorescence ima- ges and SPECT images were obtained from the same model and a contrast study of these two kinds of images was carried on by MATLAB computer platform. The shapes and cell edge distributions of the lung tumor were probed into by digital computation and relative analysis of these two kinds of images. The data measured by various methods were distributed in- to fluorescence group and SPECT group that were analysed as paired-sample. RESULTS The perimeters of tumor fluo rescence images and that of SPECT images were abnormal distribution (z= 2.12, P= 0. 034), and the difference had re- markable statistical meaning. The areas of tumor fluorescence images and that of SPECT images were normal distribution (t=165.75, P〈0. 001), and the difference had statistical significance. The areas and perimeters of tumor fluorescence im- ages were little smaller than that of the SPECT image. This contrast study showed that the edge of the SPECT image had unevenly distributed peak value of radiation intension and the edge of the fluorescence image with less tumor cells might be unreliable. It was found that the cytological migration behavior manifested complex nonlinear gradually reducing relation- ship from the center to the edge of the tumor. CONCLUSIONS The cell contrast study of the two kinds of images from the same nude mouse model showed that SPECT imaging boundary is less clear but may reveal cytological characteristics in edge of tumor cell migration. On the contrary,the fluorescence images can not show these features. This result implied that SPECT images may be an important supplement for determining the CTV of lung tumor of nude mice.
