绝经前不同年龄段乳腺癌患者的临床特征及预后比较

摘　　要：目的分析绝经前不同年龄段乳腺癌的临床特征,并比较其预后的差异。方法回顾性收集四川省肿瘤医院1999年1月至2011年9月收治的729例绝经前乳腺癌患者的临床资料及随访资料。按年龄分为35岁以下组131例、36～45岁组402和46岁以上组196例。结果在初潮年龄、活产次数和体重指数方面3组差异有统计学意义,临床特征、5年无病生存率（DFS）和5年总生存率（OS）3组的差异均无统计学意义,按分子亚型进行分层分析,绝经前三阴型乳腺癌的5年无病生存率分别为85.7%、88.7%和76.2%（P〈0.05）。临床分期为早期,淋巴结≤3个、肿瘤大小≤2 cm和luminal A型是OS的预后因素。结论年龄没有作为绝经前乳腺癌预后因素的价值,高龄的三阴型绝经前乳腺癌预后差。Objective The study analyzed the clinical characteristics in premenopausal breast cancer patients of different age groups and compared their differences in prognosis. Methods Clinical and follow-up data of 729 premenopausal patients with breast cancer hospitalized at the Sichuan Cancer Hospital between January of 1999 and September of 2011 were retrospectively collected.The patients were divided into a ≤35 age group（131 cases）, a 36-45 age group（402 cases）, and a ≥46 age group（196 cases）.Results Differences in the age of menarche, number of live birth, and body mass index（BMI） were statistically significant among the three groups; whereas differences in clinical characteristics, 5-year disease free survival（DFS）, and 5-year overall survival（OS）were not statistically significant among the three groups. Molecular subtype-based stratified analysis showed that the 5-year DFS of triple negative breast cancer（TNBC） in premenopausal patients in the three groups were 85.7%, 88.7%, and 76.2%（P=0.025）,respectively. The prognostic factors for OS among the patients were early stage of cancer, ≤3 lymph nodes, tumor size of ≤2 cm,and luminal A subtype. Conclusion Age has no clinical value as being a prognostic factor for premenopausal breast cancer, and premenopausal TNBC patients of older ages have poor prognosis.
