摘　　要：目的 探讨食管间质组织中肝素结合性表皮生长因子样生长因子（HB-EGF）和骨形态发生蛋白-4 （BMP-4）在Barrett食管发病中的意义.方法 116例患者根据胃镜下表现和送检标本HE染色结果进行分组.分为对照组（29例）、反流性食管炎（RE）组（32例）、Barrett食管组（35例）、RE治疗组（10例）和Barrett食管治疗组（10例）.采用免疫组织化学法对各组食管黏膜病变组织中尾侧型同源转录因子2（CDX-2）、HB-EGF和BMP-4的表达情况进行检测.应用方差分析比较各组间CDX-2、HB-EGF和BMP-4阳性表达水平的变化,卡方检验分析此三者表达的关系.结果 对照组、RE组和Barrett食管组的CDX 2阳性表达细胞数[（0.0±0.0）/高倍视野（HPF）、（43.1±10.6）/HPF、（67.8±11.3）/HPF]依次升高,三组间差异有统计学意义（F=67.664,P＜0.01）;此三组的HB-EGF阳性细胞数[（6.4±1.4）/HPF、（39.4±13.5）/HPF、（55.8±13.9）/HPF]和BMP-4阳性细胞数[（0.0±0.0）/HPF、（22.6±6.4）/HPF、（25.1±10.3）/HPF]也呈现相同趋势,三组间差异均有统计学意义（FHB-EGF=22.925;FBMP-4=10.463,P均＜0.01）,且除RE组与Barrett食管组的BMP-4表达外,其他各组间两两比较,差异均有统计学意义（LSD检验,P均＜0.05）.CDX-2、HB-EGF和BMP-4在RE治疗组的表达[（21.7±1.7）/HPF、（16.6±5.0）/HPF、（9.2±1.0）/HPF]以及在Barrett食管治疗组的表达[（51.4±8.7）/HPF、（31.0±10.4）/HPF、（12.7±3.9）/HPF]分别低于其在RE组和BE组的表达,差异均有统计学意义（LSD检验,P均＜0.05）.在RE组中,CDX-2的阳性率[31.2％（10/32）]明显低于HB-EGF和BMP-4的阳性率[62.5％（20/32）,56.2％（18/32）],差异有统计学意义（x2=6.275、4.063,P均＜0.05）.但在Barrett食管组中,CDX-2的阳性率[85.7％（30/35）]与HB-EGF、BMP-4的阳性率[88.6％（31/35）,74.3％（26/35）]差异无统计学意义（P均＞0.05）.结论 CDX-2、HB-EGF和BMP-4可能在RE和BarObjective To explore the function and significance of caudal type homeobox transcription factor-2 （CDX）-2,heparin-binding epidermal growth factor-like growth factor （HB-EGF） and bone morphogenetic protein 4 （BMP-4） of esophageal stromal tissues in the pathogenesis of Barrett＇s esophagus （BE）.Methods A total of 116 patients were divided into groups according to gastroscopic finds and hematoxylin-eosin （H ＆E） staining of biopsy samples.They were divided into control group （n=29）,RE group （n=32）,BE group （n=35）,RE treatment group （n=10） and BE treatment group （n=10）.The expression of CDX 2,HB-EGF and BMP-4 in different esophageal mucosal lesions was detected by immunohistochemical staining and the changes of positive expression levels of CDX-2,HBEGF and BMP 4 were compared among groups.Variance analysis and chi-square analysis were performed to analyze the correlation among the three factors.Results The CDX-2 positive cell number of control group （（0.0±0.0）/high power field（HPF））,RE group （（43.1±10.6）/HPF）,and BE group （（67.8±11.3）/HPF） increased in turn,and the differences among three groups were statistically significant （F=67.664,P＜0.01）.The HBEGF positive （（6.4±1.4）/HPF,（39.4±13.5）/HPF,（55.8±13.9）/HPF） and BMP 4 positive （（0.0±0.0）/HPF,（22.6±6.4）/HPF and （（25.1± 10.3）/HPF） cell number of three groups had the same trend and the differences among three groups were statistically significant （F HB-EGF =22.925,FBMP-4 =10.463,both P＜0.01）.Except the expression of BMP-4 between RE group and BE group,there were significant differences between every other two groups （LSD test,all P＜ 0.01）.The expression of CDX 2,HB EGF,BMP-4 in RE treatment group （（21.7±1.7）/HPF,（16.6±5.0）/HPF and （9.2±1.0）/HPF） and BE treatment group （（51.4±8.7）/HPF,（31.0± 10.4）/HPF and （12.7±3.9）/HPF） were lower than those in RE group and BE group respectively,the differences were als
