摘  要:目的探讨多排螺旋CT诊断超急性期大面积脑梗死的临床应用价值。方法筛选260例超急性期大面积脑梗死患者作为研究对象。所有患者收治入院后立即进行头颅CT检查,所有患者确诊为超急性期大面积脑梗死患者,分析所有患者CT影像结果并进行诊断。结果 260例超急性期脑梗死患者,其中231例患者平扫可见脑实质低密度表现,142例患者可见显著的局部脑肿胀征,72例患者可见单侧大脑中动脉血管高密度表现,3例患者CT影像未见异常;根据以上CT表现进行确诊,明确诊断超急性期大面积脑梗死患者185例,提示诊断72例,未见异常3例。CT确诊率与临床证实结果比较,差异无统计学意义(P>0.05)。结论采用多排螺旋CT诊断超急性期大面积脑梗死具有较高的诊断能力,扫描成像迅速且较为方便,具有临床应用及推广价值。Objective To investigate the clinical application value of multi-detector row spiral CT in diagnosis of hyperacute massive cerebral infarction. Methods There were 260 hyperacute massive cerebral infarction patients as study subjects. All patients received immediate head CT examination in admission, and they were all diagnosed as hyperacute massive cerebral infarction. CT imaging results of the patients were analyzed for judgment. Results Among 260 hyperacute massive cerebral infarction patients, 231 cases of them had low density manifestation in brain parenchyma by plain scan, 142 cases had obviously local brain swelling, 72 cases had high density manifestation in unilateral middle cerebral artery, and 3 cases had normal CT images. Definite diagnosis of 185 hyperacute massive cerebral infarction cases, 72 prompted diagnosis cases, and 3 normal cases were made by those CT manifestations. There was no statistically significant difference between CT diagnosis and clinical outcomes (P〉0.05). Conclusion Multi-detector row spiral CT provides high diagnostic ability for hyperacute massive cerebral infarction, with quick and convenient scanning images. This method contains value for clinical application and promotion.
