游离自体大网膜预防胃食管吻合口瘘的应用体会

摘　　要：目的探讨游离自体大网膜在预防食管癌贲门癌手术胃食管吻合口瘘方面的作用,为胃食管吻合口瘘提供预防措施。方法回顾性分析本院胸外科收治并手术治疗的食管贲门癌病例120例,所有病例术中均使用自体游离大网膜包裹食管胃吻合口。结果术后2例患者因意外损伤吻合口出现吻合口瘘,其余118患者均未出现吻合口瘘,顺利出院。结论游离自体大网膜能有效预防食管贲门癌术后胃食管吻合口瘘的发生。Objective To investigate the effect of dissociative autologous omentum majus in prevention of gastroesophageal anastomotic fistula in surgery for esophageal cancer and cardiac cancer, so as to provide prevention measure for gastroesophageal anastomotic fistula.Methods A retrospecitve analysis was made on 120 cases with esophageal and cardiac cancer in chest surgery of our hospital, and all patients received dissociative autologous omentum majus to wrap gastroesophageal anastomotic stoma.Results There were 2 cases with anastomotic fistula after surgery due to accident injury on anastomotic stoma, and the other 118 cases had no anastomotic fistula and left the hospital.Conclusion Application of dissociative autologous omentum majus can effectively prevent gastroesophageal anastomotic fistula after esophageal cancer and cardiac cancer surgery.
