摘  要:目的应用3.0T高分辨率磁共振成像技术,探讨急性脑缺血患者大脑中动脉斑块负荷与缺血性脑卒中之间的关系。材料与方法 20名健康志愿者与80例急性脑缺血患者在发病1周内行头MRI、MRA及大脑中动脉斑块的HR-MR扫描(亮血3D-TOF、黑血T1WI、T2WI及Pd WI),在黑血序列T2WI上,测得大脑中动脉的管腔面积(LA)、血管总面积(TVA),并计算管壁面积(WA)及管壁标准化指数(NWI)。分析斑块负荷与缺血性脑卒中之间的关系。结果 (1)80例患者中,76例的图像符合研究要求;(2)非梗死组与正常对照组比较、梗死组与正常对照组比较,TVA、WA、NWI值均有统计学意义(P<0.05);(3)非梗死组与梗死组TVA、WA值比较,差异有统计学意义(P<0.05)。结论 3.0T磁共振黑血技术能定量测量大脑中动脉斑块负荷,大脑中动脉粥样硬化与脑梗死的发生密切相关:急性脑梗死组患者大脑中动脉斑块负荷高于非梗死组。[Objective]To study the correlation between the middle cerebral artery plaque burden and recent ischemic stroke by using high resolution MR imaging at 3.0 tesla. [Methods]80 patients with acute ischemic cerebrovascular events and 20 health persons were underwent brain artery MRI imaging examination within 1week after stroke onset The total vessel area(TVA), lumen area(LA) in T2 WI were measured, then calculate the wall area(WA) and normalized wall index(NWI), then study the relationship between plaque burden and ischemic stroke. [Results](1)76/80 cases were involved in this study due to with excellent imaging quality.(2)TVA,WA and NWI between cerebral infarction and control group, without cerebral infarction and control group were significant discrepancy(P〈0.05),TVA and WA between cerebral infarction and without cerebral infarction were significant difference(P〈0.05). [Conclusion]The qualitative and quantitative analysis is available by using 3.0 T MRI, middle cerebral atherosclerosis was closely related to the ischemic cerebrovascular events, the middle cerebral artery plaque burden is higher in cerebral infarction than without cerebral infarction.
