摘 要:目的探讨中老年人颈围与血液生化指标的相关性,为血液生化指标异常的筛查探索更简便的方法。方法选择成都市参加大型免费体检的742名城乡中老年居民作为研究对象,进行体格测量、问卷调查以及血液生化指标测定。按照本课题组前期研究的颈围对肥胖的最佳预测切点(男性为37.05 cm,女性为32.73 cm)将研究对象分为颈围增大组(400名)和颈围正常组(342名),探讨颈围与血液生化指标的关系。结果不同性别人群颈围增大组的腰围、体质指数(BMI)、体重、腰臀比和小腿围均大于颈围正常组,差异均有统计学意义(P〈0.01)。颈围增大组空腹血糖、甘油三酯和极低密度脂蛋白胆固醇水平高于颈围正常组,高密度脂蛋白胆固醇水平低于颈围正常组,差异均有统计学意义(P〈0.05,P〈0.01)。另外,男性老年人群颈围增大组血红蛋白水平高于颈围正常组,差异有统计学意义(P〈0.05)。多因素logistic回归分析结果显示,男性中老年人群空腹血糖水平(OR=1.24,95%CI:1.07-1.45)、血红蛋白水平(OR=1.04,95%CI:1.01-1.06)与颈围呈正相关,高密度脂蛋白胆固醇水平(OR=0.07,95%CI:0.02-0.18)与颈围呈负相关(P〈0.05,P〈0.01),女性中老年人群甘油三酯水平(OR=1.51,95%CI:1.01-2.18)与颈围呈正相关,高密度脂蛋白胆固醇水平(OR=0.33,95%CI:0.16-0.65)与颈围呈负相关。结论颈围的测量有利于筛查发现血生化异常人群。Objective To explore the relationship between neck circumference (NC) and blood biochemical indicators among the middle-aged people. Methods A total of 742 middle-aged residents from the urban and suburban in Chengdu were served as the subjects. The investigation was conducted with anthropometric measurements, questionnaire and blood biochemical test. According to our previous study, the best cut-off values of NC were 37.05 cm tor males, 32.73 cm for females. On the basis of the best cut-off values, the subjects were divided into normal NC group (342 cases) and enlarge NC group (400 cases). The relationship between NC and blood biochemical indicators was analyzed. Results The waist circumference (WC), BMI, weight, waist-hip ratio and calf circumference (CC) in enlarge NC group were significantly higher than those in normal NC group for males and females (P〈0.01 ). The levels of FPG, TG, VLDL-C in enlarge NC group were significantly higher than those in normal NC group, and the level of HDL-C in enlarge NC group was significantly lower than that in normal NC group (P〈0.05). The level of Hb in enlarge NC group was significantly higher than that in normal NC group for males (P〈0.05). The multivariate logistic regression analyses showed that FPG (OR=1.24, 95%CI: 1.07-1.45) and Hb (OR=1.04, 95%CI: 1.01-1.06) were positively correlated with NC, HDL-C level (OR=0.07, 95%CI: 0.02-0.18) was negatively correlated with NC for males (P〈0.05); TG level (OR=1.51, 95%CI: 1.02-2.18) was positively correlated with NC, HDL-C (OR=0.33, 95%CI: 0.16-0.65) was negatively correlated with NC for females. Conclusion NC measurement is useful for screening the population with abnormal blood biochemical indicators.
