基于Meta分析血清CA724鉴别诊断胃癌与胃良性疾病的价值

摘　　要：目的 评价血清CA724鉴别诊断胃癌与胃良性疾病的价值.方法 检索中文数据库（含万方、维普、中国知网）,检索时间为2012 ～2014年发表的中文文献,收集关于检测血清CA724诊断胃癌的相关文献,进行信息统计和科学性评估;采用meta-Disc1.4软件进行数据分析;通过汇总敏感度、特异度、汇总似然比及汇总受试者工作特征曲线（SROC曲线）等统计指标综合评价检测血清CA724对胃癌的鉴别诊断价值.结果 共纳入9个研究包括2048例研究对象,所有样本均行组织病理学检查,其中确诊为胃癌的样本量为1126.汇总的敏感度和特异度分别为42％ （39％ ～45％）和94％（92％～95％）;合并受试者工作特征曲线（SROC）的曲线下面积为0.8769（0.7256～1）.结论 CA724是鉴别诊断胃癌与胃良性疾病的可靠指标.Objective To evaluate the differential diagnostic value of serum CA724 on gastric cancer and gastric benign diseases. Methods Chinese medical databases （2011-2014） were searched to collect articles in Chinese which evaluated the diagnostic value of CA724 in patients with gastric cancer. The data were analyzed by using Meta-Discl. 4 software. Meta-analysis of the reported sensitivity and specificity of each study and summary receiver operating characteristic （ SROC ） curve were performed. Results A total of 9 researches involving 2048 patients were included. The pooled sensitivity and specificity were 42% （39% - 45% ） and 94% （92% 95% ）. The area under the SROC curve was 0. 8769（0. 7256 -1 ）. Conclusion CA724 can be regarded as a valuable marker in the differential diagnosis of gastric cancer and gastric benign diseases.
