摘 要:目的探讨脑组织中水通道蛋白-4(Aquaporin-4,AQP-4)在重症急性胰腺炎大鼠中的表达及其对血脑屏障通透性的影响。方法 60只健康成年的S-D大鼠,雌雄各半,按数字表法随机分成对照组(30只)和SAP组(30只)。采用逆行胰胆管注射5%的牛黄胆酸钠(0.1 ml/100 g)建立SAP模型组,对照组以等容积无菌生理盐水代替牛黄胆酸钠,建模后12 h、24 h、48 h分批处死大鼠。1测脑组织干/湿比重;2伊文思蓝测定血脑屏障通透性;3ELISA检测脑组织中AQP-4蛋白及TNF-α的表达;4RT-PCR测定脑组织中的AQP-4 m RNA表达;5HE染色观察胰腺病理。结果 SAP组大鼠各时间点脑组织含水量明显高于对照组(P<0.05)。与对照组相比,各时间点SAP组脑组织EB水平明显升高(P<0.05),24 h达到峰值,48 h时仍维持在较高水平。SAP组大鼠脑组织AQP-4 m RNA表达增加(P<0.05)。SAP组AQP-4蛋白表达增加,与血脑屏障通透性正相关(r=0.757)。TNF-α表达与AQP-4正相关(r=0.932)。SAP组大鼠胰腺病理学评分升高(P<0.05)。结论 AQP-4增加SAP大鼠血脑屏障通透性,在SAP大鼠脑水肿的发生发展中起重要作用。Objective To investigate the expression of Aquaporin-4( AQP-4) in rats with severe acute pancreatitis and its effects on the permeability of blood-brain barrier. Methods Sixty healthy adult Sprague-Dawley rats,male and female in half,were randomly divided into control group( n = 30) and SAP Group( n = 30). 5% sodium taurocholate(0. 1 ml /100 g body weight) was injected into the pancreatic duct in the SAP group,the same amount of normal saline was used in the control group. 10 rats were killed according to the following time 12 h,24 h and 48 h respectively. The brain ratio of dry weight / wet weight( D / W) were tested. The permeability of blood brain barrier were tested by Evans blue. The content of TNF-alpha and AQP-4 in brain tissue were examined by ELISA. The expression of AQP-4 m RNA were examined by quantitative reverse transcription polymerase reaction. Pathological changes of pancreas were observed by HE staining.Results The ratio of D / W weight of brain in SAP groups were higher than that of control group( P 〈0. 05). The BBB permeability began to rise from 12 hour in SAP group,peak within 24 hour,48 hour was still at the higher level. Compared with the control group,BBB permeability increased significantly in SAP group( P 〈0. 05). The expression of AQP-4 m RNA in brain of SAP group were higher than that of control group( P 〈0. 05). The correlation analysis showed a positive correlation between AQP-4 protein expression and BBB permeability( r = 0. 757),while also between the expression of TNF-alpha in brain of SAP group and AQP-4( r = 0. 932). In the SAP group,the pathological scores increased significantly than that of the control group( P 〈0. 05). Conclusion AQP-4 increased the permeability of blood brain barrier and played an important roles in the pathogenesis of SAP of rats.
