摘  要:目的探讨缺氧诱导因子lα(hypoxia inducible factor 1 alpha,HIF-1α)、激活的STAT蛋白抑制因子y(protein inhibitor of activated STAT,PIASy)在乳腺癌中的表达及相互关系,寻找乳腺癌理想生物标志物。方法收集台州市中心医院2012年2月—2014年3月的标本分成3组:60例乳腺癌组织、20例乳腺纤维腺瘤组织及15例癌旁正常乳腺组织标本,应用免疫组织化En Vision法检测HIF-1α和PIASy的表达,应用SPSS 17.0进行统计分析,用χ2检验和Spearman等级相关分析。结果乳腺癌中HIF-1α和PIASy的阳性表达率分别为61.7%(37/60)和66.7%(40/60),显著高于乳腺纤维腺瘤组织、正常对照组的20%(4/20)、20%(3/15)和20%(4/20)、13.3%(2/15)(P<0.01),而其在纤维腺瘤组织与癌旁正常乳腺组织中阳性表达率差异无统计学意义(P>0.05)。HIF-1α和PIASy在乳腺癌组织中的表达呈正相关(r=0.500,P<0.05)。两者的表达与乳腺癌病理分级、TNM分期以及淋巴结转移有关(P<0.05),而与年龄、PR表达、HER2表达无关(P>0.05);HIF-1α的表达与ER表达无关(P>0.05),PIASy的表达与ER表达有关(P<0.05)。结论 HIF-1α和PIASy对于乳腺癌的早期诊断、评估恶性程度及转移等有着重要的临床意义,可以作为乳腺癌生物标志物。Objective To explore the expression of serum hypoxia-inducible factor-1 α (HIF-1 α) and protein inhibitor of activated STAT(PIASy) in breast carcinoma and their relationship, and search for better breast cancer biomarkers. Methods The samples were selected from Taizhou Central Hospital between February 2012 and March 2014, which divided into three groups: 60 cases of breast cancer tissue ,20 cases of breast fibroadenoma tissue and 15 cases of adjacent normal tissue. Immunohistoehemieal staining( En Vision method) was used to determine the expression of HIF-1α and PIASy. Data were analyzed using X^2 test and spearman rank correlation analysis by SPSS 17.0 version program. Results There was no significantly different immunostaining for HIF-1α and PIASy observed in breast fibroadenoma and adjacent normal tissues (P 〉 0.05 ). The positive expression rates of HIF-1 α and PIASy in breast carcinoma were 61.7 % (37/60) and 66. 7 % (40/60), which were higher than 20% (4/20) and 20% (3/15) in breast fibroadenoma, and 20% (4/20) and 13. 3 % (2/15) in adjacent normal tissues (P 〈 0.05 ). The positive relationship was found between HIF-1α and PIASy (X2 = O. 500,P 〈 0.01 ). The expression of HIF-1α and PIASy was correlated with histological grade,TNM stage and lymph node metastasis( P 〈 0.05 ). Both of them had no relevance to the age of patients and the expression levels of PR and HER2 ( P 〉 0.05 ) ; the expression of HIF-1α had no relevance to the expression of ER ( P 〉 0.05 ) , the expression of PIASy was correlated with the expression of ER( P 〈 0.05 ). Conclusion HIF-1α and PIASy have clinical significance in tumor diagnosis, malignancy prediction, tumor recurrence and metastasis, and can be used as breast caneerbiomarkers.
