颌内动脉栓塞术与鼻内镜下鼻腔电凝止血术治疗鼻出血合并脑梗死的随机对照研究

摘　　要：目的探讨颌内动脉栓塞术与鼻内镜下鼻腔电凝止血术治疗鼻出血合并脑梗死的临床疗效。方法收集2005-01—2014-01间我院收治的鼻出血合并脑梗死患者32例,随机分为观察组与对照组各16例。观察组采用颌内动脉栓塞术治疗,对照组采用鼻内镜下鼻腔电凝止血术治疗,比较2组的临床疗效与并发症。结果观察组治疗总效率为93.75%,显著高于对照组的75.00%(P<0.05);观察组的住院时间较对照组显著缩短,复发率仅为6.25%,较对照组的31.25%显著降低(P<0.05);观察组并发症发生率为37.50%,显著低于对照组的62.50%(P<0.05)。结论颌内动脉栓塞术治疗鼻出血合并脑梗死可快速有效止血,疗效优于鼻内镜下鼻腔电凝止血术,术后康复更快,安全性好,值得推广应用。Objective To investigate the clinical efficacy of the maxillary artery embolization and endoscopic nasal coagula‐tion in the treatment of epistaxis complicated with cerebral infarction. Methods 32 cases with epistaxis and cerebral infarction in our hospital from January 2005 to January 2014 were collected and randomly divided into observation group and control group ,16 cases in each group. The observation group received maxillary artery embolization ,the control group was given en‐doscopic nasal coagulation treatment ,the clinical efficacy and complications of the two groups were compared.Results The to‐tal efficiency of the observation group was 93.75% ,significantly higher than the 75.00% of control group （P〈 0.05）;the hospital stays of observation group were significantly shorter than that of the control group ,the recurrence rate in observation group was 6.25% ,significantly lower than 31.25% in the control group （P〈0.05）;the incidence of complications in the ob‐servation group was 37.50% ,significantly lower than the 62.50% in the control group （P〈0.05）.Conclusion Maxillary ar‐tery embolization in the treatment of epistaxis complicated with cerebral infarction can quickly and effectively stop bleeding , which should be widely applied with its better efficacy ,faster recovery and less complication.
