侧脑室-腹腔分流术治疗脑积水临床观察

摘　　要：目的探讨侧脑室-腹腔分流术治疗脑积水的临床疗效。方法将符合标准的55例脑积水患者,随机分成观察组(28例)和对照组(27例),观察组采用侧脑室-腹腔分流术进行治疗,对照组采用常规开放手术进行治疗。比较2组患者手术情况、疗效及并发症的发生情况。结果观察组患者的手术时间、排气时间、住院时间及Karnofsky评分分别为(12.08±1.49)min、(1.56±0.76)d、(9.6±1.6)d及(73.85±11.44),均显著优于对照组(21.25±6.86)min、(2.81±0.98)d、(11.7±2.3)d及(62.70±9.11)(P<0.05)。治疗后观察组中评分为3级的几率为67.9%,显著高于对照组的40.7%(P<0.05);观察组中评分为-1级的几率为3.6%,显著低于对照组的22.2%(P<0.05)。观察组患者远端梗阻的发生率为7.1%,显著低于对照组的29.6%(P<0.05)。结论侧脑室-腹腔分流术是一种治疗脑积水的有效手术方法,其安全性好,疗效可靠,值得临床进一步推广。Objective To investigate the clinical curative effect of ventricle‐peritoneal shunt for hydrocephalus. Methods 55 patients with hydrocephalus were randomly divided into observation group （28 cases） and control group （27 cases）. Patients in observation group received the treatment of lateral ventricle‐peritoneal shunt surgery ,while others received the treatment of open surgery. The surgery situation ,curative effect and complications were compared.Results The operation time ,exhaust time ,length of stay and Karnofsky score in observation group were （12.08 ± 1.49） min ,（1.56 ± 0.76） d ,（9.6 ± 1.6） d and 73.85 ± 11.44 ,which were significantly better than those of the control group ,which were （21.25 ± 6.86） min ,（2.81 ± 0.98） d ,（11.7 ± 2.3） d and 62.70 ± 9.11 （P〈0.05）. After the treatment ,the rate of grade 3 in observation group was 76.9% , which was significantly higher than 40.7% in control group （P〈 0.05）. The rate of grade -1 in observation group was 3.6% ,which was significantly lower than 22.2% in control group （P〈0.05）. The rate of distal obstruction in observation group was 7.1% ,which was significantly lower than 29.6% in control group （P〈0.05）. Conclusion The lateral ventricle‐peritoneal shunt surgery is an effective treatment for hydrocephalus. It is worthy of further study in clinic.
