摘  要:目的:探讨超声乳腺影像报告与数据系统( BⅠ-RADS)分级在乳腺小肿块诊断中的应用价值。方法入组行超声检查且获得病理诊断的604例乳腺小肿块(最大径≤2 cm)患者,术前按BⅠ-RADS分级标准进行描述和分级,评估超声BⅠ-RADS3~5级的阳性预测值。结果604例患者中,超声BⅠ-RADS3、4和5级阳性预测值分别为3.5%(17/483)、20.2%(22/109)、91.7%(11/12),其中≤40岁组的BⅠ-RADS 3和4级的阳性预测值低于>40岁组(2.4% vs 7.0%,12.5% vs 35.1%,P均<0.05)。结论超声BⅠ-RADS分级能有效预测乳腺小肿块的良恶性,对>40岁患者的BⅠ-RADS 3和4级乳腺小肿块要更加重视。Objective To evaluate the value of ultrasound breast imaging reporting and data system (BI-RADS) category in the diagnosis of breast cancer. Methods The 604 patients showed small breast masses that categorized BI-RADS 3-5 were pathologically confirmed, the positive predictive value of BI-RADS 3,4, and 5 were calculated. Results Among the 604 patients, the positive predictive value of each BI-RADS category was 3.5% (17/483) in category 3,20.2% (22/109) in category 4,and 91.7% (11/22) in category 5. The positive predictive value of category 3 and 4 in the ≤ 40 years group were lower than those of the 〉 40 years groups (2.4% vs 7.0% ,12.5% vs 35.1%, P 〈0. 05 ). Conclusion Ultrasound BI-RADS category is useful for predicting malignance of small breast masses, and more attention should be paid to the 〉 40 years group that categorized BI-RADS 3 and 4.
