基质金属蛋白酶-2及组织抑制因子在结肠腺瘤癌变过程中的表达及意义

摘　　要：目的探讨基质金属蛋白酶2(MMP-2)及基质金属蛋白酶抑制剂(TIMP-2)在结肠腺瘤、结肠癌及正常结肠黏膜组织中的表达及其意义。方法采用免疫组化法检测MMP-2及TIMP-2在正常大肠黏膜、结肠腺瘤及腺瘤癌变组织中的表达情况。结果 MMP-2及TIMP-2在正常结肠黏膜、结肠腺瘤组织及结直肠癌组织中均有表达,其中结直肠癌组织MMP-2的表达量明显高于正常组织及腺瘤组织,差异有统计学意义(P<0.05),腺瘤性息肉中高级别上皮内瘤变组的MMP-2表达量高于低级别组(P<0.05)。TIMP-2在腺瘤组织与正常组织中的表达无差别,在结肠腺癌组织中的表达较正常组织中降低(P<0.05)。结论 MMP-2及TIMP-2在结肠腺瘤癌变过程中分别呈高表达、低表达,二者表达的失衡在正常黏膜-腺瘤-癌的病理形成过程中起一定作用。Objective To investigate the expression and significance of matrix metalloproteinases 2 （MMP-2） and tissue inhibitor of metalloprotease 2 （TIMP-2） in colonic adenoma, colonic carcinoma and nor- mal colonic mucosa tissues. Methods The expressions of MMP-2 and TIMP-2 were measured using immuno- histoehemical staining in normal colonic mucosa, colonic adenoma and carcinoma tissues. Results MMP-2 and TIMP-2 were expressed in normal colonic mucosa, colonic adenoma and colorectal carcinoma tissues. The expression of MMP-2 in colorectal carcinoma tissue was significantly higher than that in normal tissue and ade- noma tissue （ P 〈 0.05 ）. For the adenomatous polyps with intraepithelial neoplasia, MMP-2 expression in the high grade group was higher than that in the low grade group （P 〈 0.05 ）. TIMP-2 expression in adenoma tis- sue had no difference with that in normal tissue, whereas it is lower in colonic adenoearcinoma tissue compared with normal tissue （P 〈 0.05 ）. Conclusion The expression levels of MMP-2 and TIMP-2 in the process of colonic adenoma canceration are increased and decreased respectively. And this imbalance may play a role in the progression from normal mucosa to adenoma and to cancer.
