甲氨蝶呤介入结合中药方剂灌肠治疗异位妊娠的临床疗效观察

摘　　要：目的：探讨甲氨蝶呤介入结合中药方剂灌肠治疗异位妊娠的临床效果及安全性。方法选择异位妊娠患者220例，按照随机数表法分为观察组（110例）与对照组（110例），观察组采用甲氨蝶呤介入联合中药方剂灌肠治疗，对照组单纯采用甲氨蝶呤介入治疗，比较两组患者的临床疗效及不良反应发生情况。结果观察组显效74例，有效30例，无效6例，总有效率为94．5％，对照组显效44例，有效50例，无效16例，总有效率为85．4％，组间差异有统计学意义（χ^2＝5．05，P ＜0．05）；两组治疗后血清β-HCG 等指标均明显改善，观察组改善更加明显，组间差异有统计学意义（t ＝3．05、12．93、10．92、10．50，均 P ＜0．05），观察组盆腔积液、包块消失时间及住院时间明显短于对照组，组间差异有统计学意义（t ＝7．27、8．97、8．50，均 P ＜0．05）；不良反应主要为口腔溃疡、胃肠道反应、轻度肝功能异常，两组不良反应发生率差异无统计学意义（P ＞0．05）。结论甲氨蝶呤介入结合中药方剂灌肠治疗异位妊娠的疗效确切、起效快、不良反应少。Objective To study the effect of methotrexate intervention combined with herbal enema in the treatment of ectopic pregnancy.Methods 220 cases with ectopic pregnancy were selected and divided into two groups according to the methods of random number table：the observation group and the control group,each group of 110 cases.The observation group used methotrexate intervention combined with herbal enema treatment,the control groups only used methotrexate intervention treatment.The clinical effect and adverse reactions of the two groups were observed.Results The observation groups：74 cases with marked effect,30 cases were effective,6 cases without effect,the total efficiency was 94.5%;The control groups：44 cases with marked effect,50 cases with effective, 16 cases without effect,the total efficiency was 85.4%,the difference between the two groups was statistically signifi-cant （The total effective rate χ^2 =5.05,P 〈0.05）.The index of serum β-HCG in both groups were significantly improved after treatment,the improvement in the observation group was more obvious,the difference was statistically significant （monitoring after treatment3dt =3.05,7dt =12.93,10dt =10.92,14dt =10.50,all P 〈0.05）.The hospi-talization time and the disappearance time of pelvic effusion and mass in the observation group were significantly shor-ter than that in the control group,the difference was statistically significant （Pelvic effusion absorption time t =7.27, bag piece disappear time t =8.97,hospitalization time t =8.50,all P 〈0.05）.The main adverse reactions were oral ulcers,gastrointestinal reaction,mild abnormal liver function,the two groups had no significant difference in the inci-dence of adverse reactions,without statistical significance （P 〉0.05）.Conclusion Methotrexate intervention com-bined with herbal enema in the treatment of ectopic pregnancy can obtain fast and exact curative effect,itˊs less ad-verse reaction and worthy of clinical promotion.
