象皮生肌膏对大鼠肛瘘创面修复中TGF-β_1表达的影响

摘　　要：目的探讨象皮生肌膏对肛瘘术后创面修复的作用机制。方法将30只SD大鼠肛瘘术后创面造模,随机分为象皮生肌膏组、湿润烧伤膏组和凡士林组,每组10只,分别给予相应药物创面换药。动态观察各组治疗后创面修复情况及肉芽组织中转化生长因子β1(TGF-β1)的表达情况。结果象皮生肌膏组第8天、第14天创面愈合率及TGF-β1表达水平均明显高于其他2组(P均<0.05),第14天创面愈合率湿润烧伤膏组高于凡士林组(P<0.05);创面水肿评分明显低于其他2组(P均<0.05),而湿润烧伤膏组低于凡士林组(P<0.05);创面完全愈合时间明显短于其他2组(P均<0.05),而湿润烧伤膏组短于凡士林组(P<0.05)。结论象皮生肌膏可明显提高肛瘘术后创面愈合率,缩短创面愈合时间,其促进肛瘘术后创面愈合的作用机制可能与调节创面中TGF-β1表达有关。Objective It is to discusses the mechanism of Xiangpi ShengJi ointment in the wound repair of anal fistula.Methods 30 SD rats were selected to establish the models of postoperative wound of anal fistula, then they were randomly divided into 3 groups, respectively as Xiangpi ShengJi ointment group, moist burn cream group and vaseline group, each group had 10 rats and were given respective drugs for dressing changes. The wound repair and the expression of TGF -β1 in granulation tissue in every group were observed. Results The curing rate of wound and the expression rate of TGF - β1 in Xiangpi ShengJi ointment group on the 8th and 14th day were significantly higher, while wound edema scores were lower, wound healing time was shorter than those in the other two groups （P 〈 0.05） , and on the 14th day, the indexes above including curing rate of curing rate, wound edema scores, wound healing time in moist burn cream group were better than that in vaseline group. Conclusion Xiangpi ShengJi ointment could obviously increase wound healing rate after anal fistula surgery, shorten wound healing time, and this mechanism may related with its regulation of the expression of TGF - β1 in wound tissue.
