单脐动脉脐带螺旋与胎儿畸形及围产结局的关系

摘　　要：目的研究单脐动脉(SUA)脐带螺旋指数(UCI)与胎儿畸形及围产结局的关系。方法回顾性分析重庆医科大学附属永川医院2011年1月至2013年12月经超声诊断的73例SUA胎儿资料,产后计算UCI值,与病理学检查结果对照分析。结果 SUA脐带长度正常胎儿产后平均UCI为(0.13±0.07)周/cm,螺旋稀少(UCI<0.10)发生率46.58%(34/73),其中畸形发生率35.29%(12/34),人工引产率35.29%(12/34);螺旋正常(0.10<UCI<0.30)发生率34.25%(25/73),其中畸形发生率36.00%(9/25),人工引产率40.00%(10/25);螺旋密集(UCI>0.30)发生率19.17%(14/73),其中畸形发生率(64.29%,9/14),人工引产率71.43%(10/14),胎儿畸形发生率及人工引产发病率在螺旋稀少组及螺旋正常组合并后与螺旋密集组比较,差异有统计学意义(P<0.05)。结论 SUA脐带异常螺旋密集的胎儿畸形发生率高,产前检查时应重视。Objective To analyze the relationship of umbilical coiling index（UCI）and fetus malformations and perinatal outcomes in single umbilical artery（SUA）.Methods Seventy-three SUA fetal data in the Yongchuan Hospital Affiliated to Chongqing Medical University from January 2011 to December 2013 were retrospectively analyzed,the postnatal UCI was measured and calculated,and the results were compared with pathologic findings.Results The mean UCI of normal umbilical cord length was（0.13±0.07）coils/cm in SUA.The incidence rate of hypercoiled cords was 46.58%（34/73,UCI0.1）,and the rate of malformations was 35.29%（12/34）,rate 35.29%（12/34）;34.25%（25/73）of fetals were normal UCI（0.10UCI0.30）,and the rate of malformations was 36.00%（9/25）,induced labor rate 40.00%（10/25）;19.17%（14/73）fetals were hypercoiled cords（UCI0.30）,and the rate of malformations（64.29%,9/14）and induced labor 71.43%（10/14）was the top most,which compared with hypocoiled cords and normal UCI the difference was statistically significant（P〈0.05）.Conclusion In SUA fetuses,hypercoiled cords have high incidence of abnormal fetal,which should be pay attention during the prenatal examination.
