摘  要:目的:探讨标准大骨瓣减压术治疗重型颅脑损伤的临床效果。方法选取本科2011年6月~2014年6月收治的91例重型颅脑损伤患者作为研究对象,随机分为对照组(45例)和观察组(46例),对照组采用常规骨瓣减压术治疗,观察组采用标准大骨瓣减压术治疗。比较两组术后不同时间的颅内压及总有效率。结果观察组术后第3、5、7天的颅内压显著低于对照组,差异有统计学意义(P〈0.05)。两组术后第5、7天的颅内压显著低于术后第3天,差异有统计学意义(P〈0.05)。观察组的总有效率显著高于对照组,差异有统计学意义(P〈0.05)。结论标准大骨瓣减压术治疗重型颅脑损伤效果显著,能够有效降低患者的颅内压,值得临床推广应用。Objective To explore the clinical effect of standard large bone flap decompression craniotomy in the treat-ment of severe traumatic brain injury. Methods 91 cases with severe traumatic brain injury in our department from June 2011 to June 2014 were selected and randomly divided into the control group(45 cases) and the observation group (46 cases).The control group was given conventional bone flap decompression craniotomy,the observation group was treated with standard large bone flap decompression craniotomy.The intracranial pressure at different times after surgery and the total effective rate in two groups was compared. Results The intracranial pressure of the observation group at the third days,the fifth days,the seventh days after operation was lower than that of the control group,with significant difference (P〈0.05).The intracranial pressure of two groups at the fifth days,the seventh days after operation in two groups was lower than the third days,with significant difference(P〈0.05).The total effective rate of the observation group was higher than that of the control group,with significant difference (P〈0.05). Conclusion The effect of standard large bone flap decompression craniotomy in the treatmentof severe traumatic brain injury is significant,which can effectively reduce the patient’s intracranial pressure,it is worthy of clinical promotion and application.
