2型糖尿病患者视网膜病变与尿白蛋白肌酐比值的相关性研究

摘　　要：目的探讨2型糖尿病(T2DM)视网膜病变与尿白蛋白/肌酐的比值(UACR)之间的关系。方法回顾性分析554例2型糖尿病患者的UACR与眼底摄片检查资料,根据UACR将患者分为尿白蛋白正常组(UACR<30g/mg,n=457)、微量尿白蛋白组(UACR 30-299g/mg,n=41)及大量尿白蛋白组(UACR≥300g/mg,n=56),对比三组患者一般资料差异,并对比三组患者DR发生率,探讨DR与UACR的关系。结果三组患者年龄、糖尿病病程、腰臀比、收缩压比较具有统计学差异(P<0.05或P<0.01);正常组DR发生率显著低于微量组(P<0.05),微量组显著低于大量组(P<0.05);三组患者DR发生率与UACR呈现出显著正相关(r=0.724,P<0.01)。结论对于老年、糖尿病病程较长、血压较高、腰臀比较大的T2DM患者,应加强尿蛋白及尿肌酐的监测,并及时了解其UACR,以早期发现微血管病变,并给予早期预防干预措施,防止DR的发生。Objective To discuss the relationship between UACR and DR in Patients with T2 DM.Methods UACR and eyeground radiography examination data were retrospectively analyzed in 554 patients with T2 DM,and were divided into three groups according to the UACR：the normal group（UACR30g/mg,n=457）,the trace urinary albumin group（UACR30-299g/mg,n=41）,the lot of urine protein group（UACR≥300g/mg,n=56）,then the patient＇s general information and the incidence of DR were contrasted between the three groups.Results There were significantly differences in age,diabetes duration,waist-to-hip ratio,and systolic blood pressure（SBP）between the three groups（P〈0.05 or P〈0.01）,incidence of DR of the normal group was significantly lower than the trace urinary albumin group（P〈0.05）,and the trace urinary albumin group significantly lower than the lot of urine protein group（P〈0.05）;DR incidence had a significant positive correlation with UACR in the three groups（r=0.724,P〈0.01）.Conclusion We should strengthen the monitoring of urine protein and urine creatinine in T2 DM patients with older,longer duration of diabetes,high blood pressure,larger waist-hip-ratio,and pay close attention to the UACR in time,detected the microvascular lesions early,and give early prevention and intervention measures,to prevent the occurrence of DR.
