MCM5在膀胱肿瘤组织和尿脱落细胞中的表达及临床意义

摘　　要：目的探讨微小染色体维持蛋白(MCM)在膀胱肿瘤组织和尿脱落细胞中的表达及临床意义。方法收集本院2012年8月—2013年8月住院患者及健康志愿者的尿脱落细胞或膀胱组织标本,对其进行MCM5检测。结果1膀胱癌患者在不同病理分级、临床分期MCM/β-actin的差异有统计学意义(P<0.05);280例膀胱癌患者尿脱落细胞中有71例MCM5呈阳性表达,与正常对照组和其他泌尿系疾病组比较,差异有统计学意义(P<0.05),膀胱癌患者尿脱落细胞MCM5阳性率在各病理分级、临床分期差异无统计学意义(P均>0.05),但其表达在各病理分级、临床分期差异有统计学意义(P均<0.05)。3与尿细胞学相比,尿脱落细胞MCM5检测诊断膀胱癌的灵敏度、阴性预测值较高,假阴性率低,差异有统计学意义(P<0.05),二者的特异度、假阳性率及阳性预测值比较差异无统计学意义(P>0.05)。结论尿脱落细胞中MCM5检测对膀胱癌诊断意义重大,是对膀胱肿瘤复发检测、预后评价较好的指标,值得广泛应用于临床诊断。Objective To explore the expression and clinical significance of tiny chromosome maintenance protein（MCM）in bladder cancer tissue and urine exfoliated cells.Methods Urinary exfoliated cells or tissues of the bladder of hospitalized patients and healthy volunteers in our hospital from August 2012-August 2013 were collected,and the urine exfoliated cells or tumor tissue MCM5 was tested.Results ①The difference was statistically significant of clinical stagebladder cancer patients in different pathological grading MCM/β-actin（P〈0.05）.②There were 71 cases with MCM5 positive in 80 bladder cancer patients with urinary exfoliated cells,differences between bladder cancer group and Normal control group or other urinary diseases group were statistically significant（P〈0.05）.③compared to urine cytology,sensitivity and negative predictive value of urine exfoliated cells MCM5 detection and diagnosis of bladder cancer was significantly higher,false-negative rate was significantly lower,the difference was statistically significant（P〈0.05）,There was no statistical significance difference in specific degree,false positive rate and positive predictive value（P〉0.05）.Conclusion Diagnosis of MCM5 in urine exfoliated cells to detect bladder cancer have a greater sense,it is a recurrence of bladder cancer detection,prognosis better indicator,it is worth widely used in clinical diagnosis.
