经筋针刺治疗脑卒中后痉挛性瘫痪随机平行对照研究

摘　　要：[目的]观察经筋针刺治疗脑卒中后痉挛性瘫痪疗效。[方法]使用随机平行对照方法,将60例门诊患者按抛硬币法简单随机分为两组。对照组30例阳明针刺,取肩锅、曲池、手三里、外关、合谷、脾关、梁丘、足三里、解溪、三阴交,提插捻转、平补平泻;30min/次,1次/d。治疗组30例经筋刺,以患肢关节附近肌腱两侧压痛点为主穴,以痛为腧,斜刺或直刺进针,深度以针尖达到骨膜为宜;捻转针柄,得气后将针尖稍退,顺肌腱走向一前一后透刺,透刺过程中需不断提插捻转,针刺力度以患者耐受为宜,关节出现阵挛症状止,30min/次,1次/d。连续治疗90d为1疗程。观测临床症状、GLU、GABA、不良反应。治疗1疗程,判定疗效。[结果]治疗组痊愈7例,显效15例,有效4例,无效4例,总有效率86.67%。对照组痊愈3例,显效10例,有效10例,无效7例,总有效率76.67%。治疗组疗效优于对照组(P<0.01)。受体指标两组均有改善(P<0.05),治疗组改善优于对照组(P<0.05)。[结论]经筋针刺治疗脑卒中后痉挛性瘫痪疗效满意,无严重不良反应,值得推广。[Objective] To observe the tendons acupuncture treatment for spasm and paralysis effect. [Methods] randomized, parallel controlled method, 60 outpatients simply by flipping a coin law randomly divided into two groups. 30 patients Yangming puncture, take shoulder pot, Qu pool, hand three years, foreign relations, gu, spleen off, Liang Qiu, Zusanli, Xie River, Sanyinjiao to mention plug Twist, reinforcing-reducing techniques puncture; 30 min / times, 1 times/d. Treatment group, 30 patients with tendon needling to limb joints near the tendon tenderness on both sides of the main points in pain as the acupoints to obliquely or needle piercing method, the depth to reach the periosteum tip is appropriate. Twist the needle handle, get gas after tip instincts, along the tendon to one after the penetration acupuncture, stabbed through the process need to be constantly twisting lift and thrust, acupuncture efforts patient tolerance is appropriate, joint symptoms only appear clonic, 30min/times, 1 times/d. 90 d for a course of continuous treatment. Observation of clinical symptoms, GLU, GABA, adverse reactions. One course of treatment, to determine efficacy. [Results] The cure 7 cases, 15 cases markedly effective in 4 cases, 4 cases, the total efficiency of 86.67%. The control group recovered in 3 cases, 10 cases markedly effective in 10 cases, 7 cases, the total efficiency of 76.67%. Treatment group than the control group（P〈0-01）. Receptor indicators improved in both groups（P〈0.05）, the treatment group than the control group improved（P〈0.05）. [Conclusion] After acupuncture treatment for spasm and muscle paralysis results were satisfactory, no side effects, worthy of promotion.
