右丙亚胺联合参麦注射液对治疗急性白血病患者化疗期间心脏毒性的研究

摘　　要：目的:观察比较右丙亚胺注射液单用,参麦注射液单用及右丙亚胺联合参麦注射液对治疗急性白血病患者化疗期间心脏毒性的影响。方法:将105例已明确诊断为急性白血病的患者随机分为右丙亚胺组、参麦注射液组和联合用药组,每组各35例。每组患者均应用含有蒽环类药物的化疗方案。在此基础上,右丙亚胺组患者在化疗前30 min将右丙亚胺注射液快速静脉滴注;参麦注射液组患者用50 mg/d参麦注射液静脉滴注;联合用药组患者联合应用右丙亚胺及参麦注射液静脉滴注。比较并记录3组患者心肌酶数值变化,心电图改变及左心室射血分数的变化。结果:联合用药组患者各项心脏损害的指标均低于其他两组。结论:右丙亚胺联合参麦注射液治疗以蒽环类药物为基础的化疗方案的患者,其效果优于单纯右丙亚胺或参麦注射液治疗效果。Objective： To observe and compare effects of single Dexrazoxane injection, single Shenmai injection, and Dexrazoxane combined with Shenmai injection on cardiotoxicity for patients with acute leukemia during chemotherapy. Methods： 105 patients diagnosed with acute leukemia were randomly divided into Dexrazoxane group, Shenmai injection group, and combination group, 35 cases in each group. All the patients were given the chemotherapy with anthracycline. Besides that, the Dexrazoxane group was given Dexrazoxane injection through rapid intravenous infusion 30min before the chemotherapy ; the Shenmai injection group was in- travenously injected with Shenmai injection at 50 mg/d 30min before the chemotherapy; and the combination group was intravenously injected with Dexrazoxane combined with Shenmai injection 30min before the chemotherapy. The changes of myocardial enzyme, ECG and left ventricular ejection fraction of the three groups were compared and recorded. Results ： Each index of cardiac damages of combination group was lower than those of the other two groups. Conclusions： The effects of Dexrazoxane combined with Shenmai injection for the patients under the chemotherapy with anthraeyeline are better than those of single Dexrazoxane injection or single Shenmai injection.
