摘 要:目的了解长沙地区妇女宫颈人乳头状瘤病毒(HPV)的感染现状,并分析其与宫颈病变的相关性,为本地区妇女宫颈癌的防治提供参考依据。方法以在长沙市妇幼保健院接受宫颈HPV检查的妇女12 459例为研究对象,检查结果异常者行阴道镜检查,阴道镜检查异常者行宫颈活检,以病理结果作为诊断宫颈上皮内瘤病(CIN)的金标准。结果总HPV感染率为20.8%(2597/12459),其中高危型和低危型HPV感染率分别为18.4%(2294/12459)和4.0%(502/12459);HPV单一感染和多重感染率分别为19.25%(2398/12459)和1.60%(199/12459);HPV单一感染前5位型别是HPV52、16、58、53、81型;多重感染以HR-HPV合并其他高危型为主,其中HPV16型合并其他高危型占首位。结论长沙市不同宫颈病变人群中HPV感染主要以单一型别、高危型感染为主,HR-HPV感染有年轻化及上升趋势。Objective To understand the type and risk factors of human papilloma virus(HPV) infection of women in Changsha city to provide a scientific basis for the prevention and treatment of cervical cancer.Methods 12 459 samples were detected and genotyped with genechips in our hospital. Women with a positive HPV test were referred for colposcopy, and those with abnormal colposcopy results underwent biopsy from the most severe area. Cervical intraepithelial neoplasia was diagnosed according to the pathological results. Associations between specific HPV types and the risk of cervical intraepithelial neoplasia or worse were analyzed by logistic regression. Results Among the 12 459 samples, 20.8%(2597 cases) of them were detected positive as HPV infection. The rate of high risk human papilloma virus infection was 18.4 %(2294/12 459) and the rate of low risk human papilloma virus infection was 4.0 %(502/12 459). In total, 19.25%(2398/12 459) was infected with single HPV genotype, while 1.60%(199/12 459) was multiple human papilloma virus infected. The most five infection subtypes were HPV52, 16, 58, 53, 81. Among multiple human papilloma virus infections, HPV16 combined with other HRHPV took the first place. Conclusion Single and high-risk type of HPV infection is the main feature among the patients with cervical intraepithelial neoplasia in Changsha. The incidence of HPV infection shows a trend of younger ages and continues to increase. HPV-DNA genotyping is a necessary method for cervical cancer screen.
