胎儿腹部囊性包块的产前超声诊断与鉴别诊断

摘　　要：目的 ：分析胎儿腹部囊性包块的产前超声检查特点,降低误诊率,提高超声在产前鉴别诊断的应用价值。方法 ：对我院120例超声检查出胎儿腹部囊性包块并有随访确诊的案例进行回顾性分析,比较不同来源的囊性包块的诊断结果,分析超声声像图。结果 ：对比产前超声诊断和引产或出生后确诊结果发现,超声诊断出现8例误诊,误诊率为6.67%,主要为胆道源性囊性包块误诊2例,肝源性囊性包块误诊1例,肠道源性囊性包块误诊2例,肾源性囊性包块误诊2例,生殖系统源性囊性包块误诊1例。而腹部囊性包块首次检出时间主要在孕21-30周,胆道源性和肝源性主要集中在孕15-25周时,而肠源性、肾源性等来源的囊性包块首次检出时间主要在孕20-30周。结论 ：产前超声检查通过观察胎儿腹部囊性包块的形态、大小和血流等指标,同时结合孕周能较为准确的诊断出病变来源,对胎儿产前准确诊断具有重要意义。Objective： To analyze the results of prenatal ultrasound fetal abdominal cystic mass, reduce fetal prenatal ultrasound diagnosis of abdominal cystic mass misdiagnosis rate. Methods： 120 patients in our hospital ultrasound fetal abdominal cystic mass and a follow-up of diagnosed cases were retrospectively analyzed, the diagnosis of cystic mass comparison of different sources, analyze monographic. Results： Misdiagnosis rate was 6.67%, mainly as endogenous billiard cystic mass misdiagnosed 2 cases （2/120, 1.67%）, liver -derived cystic mass misdiagnosed one case （1 /120, 0.83%）, gut -derived cystic mass naisdiagnosed 2 cases （2/120, 1.67%）, reproductive system derived cystic mass misdiagnosed one case （1/120, 0.83%）. The abdominal cystic mass first time detected mainly in the 21 to 30 weeks gestation, billiard endogenous and exogenous mainly concentrated in the liver of 15 to 25 weeks gestation, and intestinal and kidney -derived sources such as capsular mass in the first detection time mainly in the 20 to 30 weeks gestation. Conclusion： Prenatal ultrasound fetal abdominal cystic mass by observing the shape, size and blood flow and other indicators, combined with gestational age can be more accurate diagnosis of lesions source for accurate prenatal diagnosis of fetal significance.
