摘  要:目的:建立与人类早产儿脑室周围白质软化(PVL)病理相似的可靠动物模型。方法:144只2日龄(P2)sD新生大鼠,体重6~9g,雌雄不拘,随机分成正常对照组(48只),假手术组(48只)、缺氧缺血性脑损伤(HIBD)组(48只)。P2新生大鼠麻醉后经结扎左侧颈总动脉及8%浓度的氧气缺氧4h制作HIBD组。假手术组麻醉后分离左侧颈总动脉不结扎不缺氧。正常对照组未予任何处理。缺氧缺血后3天、1周、2周、4周时,各组大鼠分别麻醉后予4℃生理盐水及4%多聚甲醛心脏灌注,脑组织于4%多聚甲醛中后固定,观察脑形态学改变,脑组织石蜡切片行HE染色,病理评分。结果:HIBD组可见白质水肿、结构疏松、左侧纹状体梗死灶形成、病灶内胶质细胞浸润,丘脑及海马可见细胞凋亡。5日龄(P5)大鼠病理评分:HIBD组与正常对照组、假手术组差异有统计学意义(P〈O.01);9日龄(P9)大鼠病理评分:HIBD组与正常对照组、假手术组差异有统计学意义(P〈0.05);16日龄(P16)大鼠病理评分:HIBD组与正常对照组、假手术组差异有统计学意义(P〈0.01);30日龄(P30)大鼠病理评分:HIBD组与正常对照组、假手术组差异有统计学意义(P〈0.01)。HIBD组评分均高于其他组,且呈上升趋势。结论:通过对P2新生大鼠麻醉后经结扎左侧颈总动脉及8%浓度的氧气缺氧4h,成功建立与人类早产儿PVL病理变化相似的动物模型。Objective: To establish a immature rat model of cerebral hypoxia-ischemia which was expected to be similar to periventricular leukomalaeia in human preterm infants pathologically and neuroethologically. Methods: 144 Sprague-Dawley rats (Postnatal day 2, P2) , of either sex, were randomly divided into 3 groups: normal-group, sham operation-group and HIBD-group. Every group had 48 rats. HIBD-group survived the left common carotid artery (CCA) ligation followed by 4 h exposure to 8% 02. Rats in sham operation- group only survived the left CCA isolation. Rats in normal-group was without doing anything. Brain tissues were collected at day 3, 1 week, 2 weeks, 4 weeks after HI, for Hematoxylin-Eosin staining and histological scoring. Results: In the HIBD-group, edema, rarefaction of structure were noticed in white matter, and focus of infarction and gliocyte infiltration were noticed in the left corpus striatum. P5 histological scoring: HIBD-group were significantly higher, compared that of normal-group, sham operation-group (5.75±2.44 vs 1.17±1.17, 0. 67±0. 82, P〈0.01 ) ; The difference in the P9 histological scoring between the HIBD-group and normal-group, sham operation-group had a statistical significance (7.00±3.42 vs 1.67±0. 52, 1.67±1.63, P〈0. 05) ; P16 histological scoring: HIBD-group was significantly higher, compared that of normal-group, sham operation-group (8. 38±1.92 vs 1.67±1.33, 1.33±1.21, P〈0. 01) ; The difference in the P30 histological scoring between the HIBD-group and normal-group, sham operation-group had a statistical significance (9. 88 ±0. 99 vs 0. 50 ±0. 84, 0. 67± 0. 82, P〈0. 01 ) . Conclusion: P2 rats survived the left common carotid artery (CCA) ligation followed by 4 h expo- sure to 8% 02 are similar to periventricular leukomalacia in human preterm infants pathologically and neuroethologically.
