摘 要:目的探讨CD8+CD28+Tc细胞在急性白血病患者化疗前后外周血中的表达及其临床意义。方法应用流式细胞术检测急性白血病初诊患者40例,化疗后完全缓解患者30例和健康体检者30例的外周血CD8+CD28+Ts细胞的表达情况。结果急性白血病初诊患者外周血中CD8+CD28+Tc细胞的表达率为(7.83±4.83)%,较健康对照组表达率[(14.92±4.13)%]明显偏低,差异具有统计学意义(P<0.01);化疗后完全缓解患者外周血CD8+CD28+Tc细胞的表达率为(8.21±5.68)%,较健康对照组明显偏低,差异具有统计学意义(P<0.01);完全缓解组外周血CD8+CD28+Tc细胞的表达率与初诊患者组比较差异无统计学意义(P>0.05);缓解组CD8+T细胞的表达率为(43.87±16.47)%,均比初诊患者组[(31.53±7.25)%]和健康对照组[(33.65±7.08)%]的表达率明显偏高,差异具有统计学意义(P<0.01);急性髓细胞白血病(AML)和急性淋巴细胞白血病(ALL)的外周血CD8+CD28+Tc细胞的表达率分别为(7.90±4.63)%和(6.47±2.86)%,均明显低于健康对照组,差异均具有统计学意义(P<0.01);CD8+CD28+Tc细胞与CD8+T细胞的直线回归相关系数r=0.098,差异无统计学意义(P>0.05),其无正相关关系。结论 CD8+CD28+Tc细胞在急性白血病初诊患者和化疗后完全缓解患者的外周血中表达明显偏低,提示其抗肿瘤能力下降,需要通过有效干预患者的免疫功能,提高机体的抗肿瘤能力,对提高急性白血病的疗效有重要意义。Objective To investigate the expression of CD8^+CD28^+Tc cell in the peripheral blood of patients with acute leukemia before and after chemotherapy and its clinical significance. Methods The expression rates of CD8^+CD28^+Tc cell in peripherical blood were detected by flow cytometry in 40 newly diagnosed patients with acute leu-kemia (group A), 30 patients with complete remission after chemotherapy (group B), and 30 healthy physical examin-ers (group C). Results The expression rate of CD8^+CD28^+Tc cell of group A was (7.83±4.83)%, significantly lower than that of group C [(14.92 ± 4.13)%, t=5.68, P〈0.01]. The expression rate of CD8^+CD28^+Tc cell of group B was (8.21±5.68)%, which was significantly lower than that of group C (P〈0.01) and not significantly different from that of group A (P〉0.05). The expression rate of CD8^+T cell in group B was significantly higher than that of group A and group C [(43.87±16.47)%vs (31.53±7.25)%and (33.65±7.08)%, P〈0.01]. The expression rate of CD8^+CD28^+Tc cells of acute myelocytic leukemia (AML) and acute lymphoblastic leukemia (ALL) were (7.90 ± 4.63)%and (6.47 ± 2.86)%, significantly lower than those of group C (P〈0.01). The linear regression correlation coefficient (r) between CD8^+CD28^+Ts cells and CD8^+T cells was 0.098, with no statistically significant difference (P〉0.05) and no positive correlation be-tween them. Conclusion The expressions of CD8^+CD28^+Tc cells in peripheral blood in newly diagnosed patients with acute leukemia and those in complete remission after chemotherapy are significantly lowered, indicating its decreased anti-tumor ability. To effectively intervene in immunity function of patients and to improve the anti-tumor ability are of great significant to improve the curative effect of acute leukemia.
