支气管镜下高频电刀术联合化疗治疗老年非小细胞肺癌的疗效观察

摘　　要：目的:观察支气管镜下高频电刀术联合全身化疗治疗老年中央型非小细胞肺癌(NSCLC)患者的疗效。方法:将38例老年中央型非小细胞肺癌患者随机分为治疗组和对照组,每组各19例。治疗组患者行支气管镜下高频电刀烧灼切割术,并给予化疗,对照组单用化疗,2组化疗方案相同。比较2组患者气促分级、Karnofsky评分(KPS)、肺功能改善程度以及近期、远期疗效。结果:治疗组气促分级、肺功能、KPS评分改善明显优于对照组。治疗组完全缓解1例,部分缓解15例,有效率明显高于对照组(84.2%vs 57.9%,P<0.05)。治疗组中位生存时间,0.5年、1年生存率明显高于对照组(P<0.05)。结论:经支气管镜下高频电刀术联合化疗可有效解除气道阻塞,提高患者生活质量,近期疗效显著,中位生存时间、1年生存率提高,值得临床推广。Objective： To observe the therapeutic effect of high frequency electric knife technique under the bronchoscope combined with systemic chemotherapy in treatment of senile non-small cell lung cancer（ NSCLC）. Methods： Thirtyeight cases of elderly patients with central type NSCLC were randomly divided into treatment group and control group,19 cases for each group. Treatment group underwent high frequency electricity knife cutting and burning under the bronchoscope combined with chemotherapy,while control group received the same chemotherapy regimens alone. Dyspnea grading,Karnofsky performance score（ KPS）,lung function improvement and the short-term and long-term therapeutic effect were compared between the two groups. Results： Grade of dyspnea,pulmonary function,KPS score improved significantly in treatment group. In 19 cases of treatment group,1 case achieved complete remission,15 cases with partial remission,the efficacy rate was significantly higher than that in control group（ 84. 2% vs 57. 9%,P〈0. 05）. The median survival time,half year and one year survival rate were significantly higher than that of control group（ P〈0. 05）. Conclusions： The bronchoscopic high frequency electric knife technique can effectively relieve airway obstruction and improve the quality of patients ＇ life. The short-term therapeutic effect is distinct,the median survival time and one year survival rate are higher than those of control group. It is worthwhile to recommend for clinical practice.
