内镜黏膜下剥离联合内镜超声在胃黏膜下肿瘤诊治中的应用

摘　　要：目的探讨内镜超声(EUS)指导内镜黏膜下剥离(ESD)切除胃黏膜下肿瘤的治疗价值。方法胃镜检查中发现135例胃黏膜下隆起型病变,均行EUS检查,排除外压性改变、血管性病变或进展期癌变(癌变累及固有肌层、浆膜层及临近或远处转移)患者34例,对101例黏膜下肿瘤患者行ESD治疗,并将EUS检查结果与术后病理结果进行回顾性对照分析。结果 101例病变均一次性完整切除,术后病理诊断为胃间质瘤57例,胃异位胰腺23例,胃脂肪瘤15例,胃神经纤维瘤3例,平滑肌瘤2例,胃癌1例。结论 EUS指导ESD治疗胃黏膜下肿瘤,可一次性完整切除病变,提供完整的病理学诊断资料,是安全、有效的微创治疗的好方法。Objective To investigate the clinical efficacy and safety of endoscopic submucosa[ dissection （ESD） associated with endoscopic ultrasonography （EUS） for the gastric submucosal tumor （SMT）. Methods A total of 135 cases of gastric eminence lesions underwent EUS, and among which 101 cases of gastric SMT were treated with ESD. The pathological speci- mens were analyzed retrospectively. Results A total of 101 SMT were rejected completely. The pathological results included 57 cases of gastric stromal tumor, 23 cases of gastric ectopic pancreas, 15 cases of gastric lipoma, 3 cases of gastric neurofibroma, 2 cases of gastric leiornyorna, and 1 case of gastric cancer. Conclusion ESD associated with EUS is an efficacious and safe proce- dure for the treatment of gastrointestinal tract diseases. It is possible to completely resect lesions and provide sufficient patholog- ical information.
