肿瘤患者大量输血后临床检验指标变化分析

摘　　要：目的观察肿瘤患者大量输血后主要临床指标数值变化,为临床防治大量输血并发症提供依据。方法肿瘤患者大量输血后71例进行血小板计数、凝血四项、肝功能、肾功能以及血清电解质进行比较分析。结果患者在大量输血后血小板计数(PLT)、纤维蛋白原(FIB)、血清总蛋白(TP)、血清钾(K+)、血清钙(Ca2+)及血清镁(Mg2+)较输血前显著降低(P<0.05);活化部分凝血活酶时间(APTT)、凝血酶原时间(PT)、凝血酶时间(TT)、丙氨酸氨基转移酶(ALT)及天冬氨酸氨基转移酶(AST)较输血前数值显著增高(P<0.05);血清总胆红素(TBIL)、尿素氮(BUN)、肌酐(Cr)、尿酸(UA)、血清钠(Na+)及血清氯(Cl-)与输血前水平无明显差异(P>0.05)。结论肿瘤患者大量输血治疗过程中应及时监测凝血功能、肝功以及电解质水平,以达到预防及治疗并发症的目的。Objective To observe the test indicators＇ changes of cancer patients who received massive blood transfusion treatment. Methods A total of 71 cancer patients that had received massive transfusion were enrolled in this study. Blood coag- ulation function, platelet numerical, hepatic function, renal function and electrolyte before and after the massive transfusion were detected and compared. Results After undergoing the massive transfusion treatment, the values of platelet （PLT）, fi- brinogen （FIB）, total protein （TP）, serum potassium （K^＋）, serum calcium （Ca^2＋） and serum magnesium （Mg^2＋） were significantly lower than pre-transfusion （P〈0.05）. The values of prothrombin time （PT）, activated partial coagulation time （APTT）, thrombin time （TT）, glutamic-pyruvic transaminase （ALT） and glutamic oxalacetic transaminase （AST） were higher than pre-transfusion （P〈O. 05）. The values of serum total bilirubin （TBIL）, urea nitrogen （BUN）, serum creatinine （Cr）, Uric Acid （UA）, serum sodium （Na＋ ） and serum chlorine （C1-） were not different between pre-transfusion and post- transfusion （P〉0.05）. Conclusion During massive transfusion treatment, blood coagulation function, platelet numerical, re- nal function and serum electrolytes should be detected to prevent and treat complication of massive transfusion.
