摘　　要：目的探讨小切口囊外摘除术和超声乳化吸出术在高龄白内障患者中的应用疗效。方法选取高龄白内障患者56例,分别行小切口囊外摘除术(A组,26例)和超声乳化吸出术(B组,30例),同时一期植入同型号硬性人工晶体。观察比较两组患者的视力、散光及术后角膜水肿。结果术后1周B组视力明显高于A组(P<0.05),术后1、3个月两组患者视力差异无统计学意义(P>0.05)。两组患者术后1个月内散光均明显高于术前(P<0.05)且术后3个月内恢复(P>0.05);术后不同时期两组间散光差异均无统计学意义(P>0.05)。术后A组角膜水肿发生率低于B组(P<0.05),两组均在术后1周内恢复。结论小切口囊外摘除术治疗高龄白内障患者,操作及设备简便,不受晶状体核硬度限制,费用低,疗效与超声乳化等同,适于基层医院推广。Objective Discuss the application and effect of manual small incision cataract surgery and cataract phacoemulsification surgery in senile cataract patients. Methods Selected 56 senile cataracts who were respectively performed manual small incision cataract surgery（group A, 26 cases）and cataract phacoemulsification surgery （group B, 30 cases）, meanwhile implanted the Hard HV Intraocular Lens. Compare visual acuity postoperative astigmatism, Postoperative corneal edema of the two groups. Results The vision of group B was obviously higher than group A （P〈0.05）in the first week after the operation. There was no visual acuity differences between the two groups in the first and third months after surgery, P〉0.0$. Astigmatism was obviously higher after operation in the first month in two groups（P〈0.05）, and restored to former levels in the third month after the operation（P〉0.05）. There was no significant difference in astigmatism between the two groups after the operation（P〉0.05）. Incidence of postoperative corneal edema of group A was lower than group B （P〈0.05）, and both groups in the first week back to normal after surgery. Conclusion The effect between manual small incision cataract surgery and cataract phacoemulsification surgery is similar, even the manual small incision cataract surgery is simpler and cheaper, so it is more suitabler for basic-level hospitals.
