摘　　要：目的探讨应用Solitaire AB支架机械取栓联合动脉溶栓治疗急性缺血性脑卒中的安全性及效果。方法选取2010年1月至2014年10月应用Solitaire AB支架机械取栓联合动脉溶栓治疗的18例急性缺血性脑卒中患者,评价其疗效和预后。结果 18例患者经支架机械取栓结合动脉溶栓后均成功获得全部或部分再通。术前与术后美国国立卫生研究院卒中量表评分相比,差异有统计学意义(P<0.05)。3个月后疗效评估,17例改良Rankin评分(mRS)显示预后良好,其中0分10例,1分6例,2分1例;1例预后不良,mRS 4分。18例患者复查均无血管再闭塞等并发症。结论应用Solitaire AB支架机械取栓联合动脉溶栓治疗急性缺血性脑卒中是相对安全、有效的,但应严格把握手术适应证。Objective To investigate the effectiveness and safety of mechanical thrombectomy with solitaire AB stents in combination with intra-arterial thrombolysis for treatment of acute ischemic stroke. Methods The clinical data of 18 consecutive patients with acute is-chemic stroke treated with mechanical thrombectomy, using Solitaire AB stents in combination with intra-arterial thrombolysis in our hospital from January 2010 to October 2014, were analyzed retrospectively. The patients were followed up for three months after the procedure. Re-sults Complete or part recanalization was achieved by mechanical thrombectomy with solitaire AB stents in combination with intraarterial thrombolysis in all eighteen patients. The National Institute of Health Stroke Scale （ NIHSS） score was obviously lower after treatment than before treatment, with a significant difference （ P〈0. 05 ） . During the follow-up period lasting three months, 17 patients had a good mRS score, including 10 cases with a mRS score of zero, 6 cases with one and 1 case with two. One patient had a mRS score of four. Re-obstruc-tion occurred in none of the patients. Conclusion It appears to be safe and effective for endovascular treatment of acute ischemic stroke by using mechanical thrombectomy with solitaire AB stents in combination with intra-arterial thrombolysis.
