摘  要:目的探讨胃癌CEUS参数与微血管密度(MVD)的相关性。方法对确诊的37例胃癌患者行术前CEUS,分别记录胃癌病变区及正常胃壁TIC曲线的峰值时间(TTP)、增强时间(ET)、曲线上升支斜率(WIR)。计数胃癌病变及其旁正常胃壁的MVD。分析CEUS的TIC参数与MVD的相关性。结果胃癌病变区TIC的ET、TTP低于其旁正常胃壁(P均<0.05),WIR、MVD高于其旁正常胃壁(P<0.01);胃癌病变区的ET、TTP与MVD呈负相关(r=-0.70、-0.59,P均<0.01),WIR与MVD呈正相关(r=0.70,P<0.01)。结论对术前经胃镜病理活检确诊的胃癌行CEUS,可以直接显示并评估肿瘤血流灌注状态,从而间接评价新生血管生成程度,可预测肿瘤的发生发展并判断预后。Objective To discuss the correlations of CEUS parameters and microvascular density(MVD)in gastric carcinoma.Methods Thirty-seven gastric cancer patients preoperative diagnosis underwent preoperative CEUS.The time to peak(TTP),enhanced time(ET),wash in rate(WIR)of time-intensity curve(TIC)of gastric carcinoma lesions and normal gastric wall were recorded.MVD in carcinoma lesions and adjacent normal gastric wall were counted.Correlations of TIC parameters and MVD were analyzed.Results The ET and TTP of gastric lesions were lower than those of normal stomach(all P〈0.05),WIR and MVD of gastric lesions were higher than those of normal stomach(all P〈0.01).ET,TTP of gastric lesions were negatively correlated with MVD(r=-0.70,-0.59,both P〈0.01),when WIR was positively correlated with MVD(r=0.70,P〈0.01).Conclusion CEUS performed before operation in gastric cancer preoperative biopsy confirmed,can display and evaluate tumors' perfusion,and indirectly evaluate the extent of neovascularization generate and predict tumors' development and prognosis.
