使用尿黄体生成素试纸和阴道超声监测排卵效果的比较

摘　　要：目的观察多囊卵巢综合征（PCOS）患者口服来曲唑促排卵后行尿黄体生成素（LH）试纸自我监测与阴道超声监测排卵的效果。方法募集PCOS患者56例,于月经第3~7天每日给予来曲唑1片（2.5mg/d）行促排卵。根据地域分布确定为尿LH试纸自我监测组（试纸组,27例）和阴道超声监测组（B超组,29例）。均于月经第10天开始监测排卵。适时指导同房。后期均给予黄体支持10d。停药7d观察是否月经来潮,早孕试纸或医院抽血查HCG,判断是否妊娠。结果两组的年龄、体重指数（BMI）、不育年限均无统计学差异（P〉0.05）。B超组29例共行77周期,试纸组27例共行68周期。B超组的排卵率与累积妊娠率分别为77.92%和58.62%,高于试纸组（64.7%、40.74%）,但差异无统计学意义（P〉0.05）。B超组的总周期妊娠率（22.07%）与尿LH试纸组（16.17%）比较,差异亦无统计学意义（P〉0.05）。结论 PCOS患者中用来曲唑促排卵后行尿LH试纸自我监测与超声监测排卵有近乎同等的效果。Objective：To compare the efficacy of between urinary LH urine test and transvaginal ultrasound for monitoring ovulation in patients with polycystic ovary syndrome（PCOS）treated with oral letrozole.Methods：The aromatase inhibitor letrozole was given orally in a dose of 2.5 mg on days 3-7of menstrual cycle in the 56 patients with PCOS.The patients were divided into two groups：the urinary LH test group（29patients）and the transvaginal ultrasound group（29patients）for monitoring ovulation.The monitoring ovulation started from the 10 th day of the menstrual cycle.Every woman was asked to have intercourse in due course.After ovulation was observed,the patients were given dydrogesterone for luteal phase support for 10 days.Then the women stopped taking dydrogesterone for 7days and measure blood HCG levels.Results：There were no significant differences in age,duration of infertility,BMI between the two groups.The ovulation rate（77.92% vs.64.7%）and cumulative pregnancy rate（58.62% vs.40.74%）in the transvaginal ultrasound group were higher than those in the LH test group,but not significant different（P〉0.05）.The total cycle pregnancy rate in the transvaginal ultrasound group（22.07%）is similar to that in urinary LH test group（16.17%）.Conclusions：Urinary LH test has similar efficacy to transvaginal ultrasound for monitoring ovulation in PCOS patients after treatment with letrozole.
