Protective Effects of Remote Ischemic Postconditioning on Blood Brain Barrier in Focal Cerebral Ischemia Reperfusion Rats

摘  要:【目的】研究在大鼠脑缺血再灌注(IR)损伤模型中远程缺血后适应对血脑屏障的作用及相关机制。【方法】将SD大鼠96只随机分为3组:假手术组、单纯缺血再灌注组和远程缺血后适应组。利用线栓法制作大鼠脑IR模型,缺血1.5 h后恢复再通,再灌注72 h后对各组大鼠进行神经功能评分,染色检测脑梗死体积、梗死侧大脑半球脑组织伊文思蓝(EB)渗出量,再对紧密连接相关蛋白Claudin-5和Occludin的表达水平进行分析。【结果】与假手术组相比,单纯缺血再灌注组神经功能评分(9.9±2.0)、脑梗死体积(58.8±1.4)及EB的渗出量(5.7±0.6)均增加(P<0.01),同时Claudin-5和Occludin表达下降(P<0.01);与单纯缺血再灌注组比较,远程缺血后适应组大鼠神经功能评分(7.5±1.7)降低(P<0.01),脑梗死体积(33±5)减少(P<0.01),EB渗出量(2.7±1.2)减少(P<0.01),相应地Claudin-5和Occludin表达增加(P<0.01)。【结论】远程缺血后适应可以减轻脑缺血再灌注损伤,维持血脑屏障稳定性,可能与增加Claudin-5、Occludin的表达有关。[Objective] To explore the protective effects of remote ischemic postconditioning to blood brain barrier (BBB) in cerebral ischemia reperfusion (IR) injury and potential mechanisms involved.[Methods] Ninety-six SD rats were divided into three groups randomly:sham operation group,ischemia-reperfusion (IR) group and remote ischemic postconditioning group (RIP).Rats were exposed to 1.5h of middle cerebral artery occlusion and then 72 h of reperfusion.Rats in remote ischemic postconditioning group were given brief occlusion and release of femoral arteries.After 72 h of reperfusion,neurological function scores,infarct volumes of brains,EB leakage of ischemia cerebral hemispheres,and the expression of tight junction (TJ) associated protein Claudin-5 and Occludin were tested and analyzed.[Results] Compared to sham group,rats in IR group showed elevated neurological function scores (9.9 ± 2.0) (P< 0.01),infarct sizes (58.8 ± 1.4) (P< 0.01),increased EB leakage (5.7-± 0.6) (P < 0.01),accompanied with the expression of Claudin-5 and Occludin down-regulated (P < 0.01) at 72h after reperfusion.Compared to IR group,rats in RIP group showed decreased neurological function scores (7.5 ± 1.7) (P< 0.01),infarct sizes (33 ± 5) (P< 0.01) and EB leakage (2.7 ± 1.2) (P<0.01),accompanied with increased Claudin-5 and Occludin expression (P < 0.01).[Conclusions] Remote ischemic postconditioning can reduce cerebral IR injury,which may be associated with up-regulating the expression of TJ associated proteins to maintain the stability of BBB.
