奈达铂、顺铂与5-氟尿嘧啶联合治疗中晚期宫颈癌患者的临床疗效观察及安全性分析

摘　　要：目的探讨奈达铂、顺铂与5-氟尿嘧啶联合治疗中晚期宫颈癌患者的疗效及安全性。方法选取2009年5月至2011年5月在湖南省肿瘤医院确诊的中晚期宫颈癌患者86例,采用随机数字表法随机均分为两组,各43例,分别行奈达铂＋5-氟尿嘧啶同步放化疗（奈达铂组）与顺铂＋5-氟尿嘧啶同步放化疗（顺铂组）,观察记录不良反应和疗效。结果奈达铂组41例有效,有效率为95.4%;顺铂组32例有效,有效率为74.4%,两组近期疗效比较差异有统计学意义（P〈0.05）,奈达铂组复发率显著低于顺铂组[7.0%（5/43）比25.6%（11/43）,P〈0.05],奈达铂组Ⅲ~Ⅳ度白细胞减少发生率显著低于顺铂组[9.3%（4/43）比32.6%（14/43）,P〈0.01]。结论奈达铂＋5-氟尿嘧啶同步放化疗比顺铂＋5-氟尿嘧啶同步放化疗具有更好的疗效,更少的不良反应。Objective To explore the therapeutic effect and safety of the combination of nedaplatin or cisplatin and 5-fluorouracil for treatment of advanced cervical cancer. Methods A total of 86 patients with confirmed diagnosis of advanced cervical cancer in Hunan Cancer Hospital from May 2009 to May 2011 were included. They were randomly divided into two groups by random number table method,43 patients in each group. The nedaplatin group was given nedaplatin plus 5-fluorouracil concurrent radiochemotherapy,the other group was given cisplatin plus 5-fluorouracil concurrent radiochemotherapy. The adverse effects and therapeutic effects were observed and recorded. Results In nedaplatin group there were 41 cases of effective,and the effective rate was 95. 4%; in cisplatin group there were 32 cases of effective,and the effective rate was74. 4%,the differences of short-term effects between the two groups were significant（ P〈0. 05）. Recurrence rate of nedaplatin group was lower than cisplatin group[7. 0%（ 5 /43） vs 25. 6%（ 11 /43）,P〈0. 05 ],Ⅲ,Ⅳ WBC decreases of nedaplatin group was lower than cisplatin group[9. 3%（ 4 /43） vs 32. 6%（ 14 /43）,P〈0. 01]. Conclusion Compared to cisplatin plus 5-fluorouracil concurrent radiochemotherapy,nedaplatin plus 5-fluorouracil concurrent radiochemotherapy has better curative effects and less adverse effect.
