双层冰毯联合冰袋物理降温治疗中枢性发热疗效观察

摘　　要：目的:考察双层冰毯联合冰袋物理降温治疗中枢性发热的临床疗效。方法:选择符合中枢性发热、体温大于38.5℃患者60例,每组各入选患者30例,分A组(n=30):双层冰毯+冰袋+常规药物治疗、B组(n=30):冰袋+常规药物治疗,记录两组患者体温达标时间,清醒后进行神经功能缺损评分,比较各组患者预后恢复情况。结果:经降温后,两组患者体温均达到降温目标(核心温度37.5℃),A组患者体温达标时间短于B组(107.7±37.1min vs 169.3±22.6min,P<0.01),神经功能缺损评分显示A组分值小于B组(11.54±4.67 vs 17.63±5.21,P<0.01),预后恢复良好比例显著提高(P<0.05)。结论:双层冰毯联合冰袋物理降温可迅速降低中枢性高热患者体温,改善患者预后恢复情况。Objective：To investigate the efficacy of double ice blanket combined with ice pack in patients with centric fever. Methods：A total of 60 individuals be up to centric fever with core temperature＆gt;38.5℃ were enrolled and divided into A group （n=30）：double ice blanket/ice pack/basic procedures and B group （n=30）：ice pack/ba-sic procedures. The time was recorded when the body temprature was decreased to the core temperature≤37.5℃and the neurological deficit scores were used to evaluate to the recovery from the sufferers＆#39; illnesses. Results：Post the treatment of temperature reduction, the two groups of patients could recuperate from fever. However, the sufferers from A group expended shorter time than ones from B group before the body temperatures decline to target point （107.7± 37.1 min vs 169.3±22.6 min,P〈0.01）. The neurological deficit scores revealed that the average value of A group no-tably lower than group B （11.54±4.67 vs 17.63±5.21,P〈0.01） and the application of double ice blanket combined with ice pack on centric fever observably alleviated the rate of disability （ P〈0.05）. Conclusion：reatment with double ice blanket combined with ice pack could effectively reduce the body temperature in hyperpyretic neurologic patients and allay the nonreversible damnification.
