摘 要:目的 观察低频脉冲电磁场(LF-PEMF)对大鼠激素性股骨头坏死(SANFH)骨组织过氧化物酶体增殖物激活受体-γ2(PPAR-γ2)及Runt相关转录因子2(Runx2)表达的影响.方法 将42只Wistar大鼠随机分为3组:(1)模型组(S,n=16),尾静脉注射10 μg/kg大肠杆菌内毒素;24 h后臀肌注射20 mg/kg甲基泼尼松龙,24h 1次,共3次.造模后不予磁场照射;(2)磁场治疗组(S+P,n=16),造模方法同前,4周后给予磁场照射(15 Hz,4.5 ms,1.2 mT),4 h/d,共8周;(3)空白对照组(C,n=10),不造模,不予磁场照射.磁场治疗8周后处死所有大鼠,取股骨头组织行苏木素-伊红(HE)染色,采用反转录酶-聚合酶链锁反应(RT-PCR)及Western blot检测骨组织内PPAR-γ2及Runx2的表达.结果 S组SANFH发病率(69%)及空骨陷窝率[(59.55±21.70)%]明显高于S+P组[31%、(36.16±15.34)%,P<0.05]及C组[0%、(8.79±2.42)%,P<0.05];S组股骨头组织内PPAR-γ2 mRNA及蛋白的表达显著高于S+P组及C组,差异有统计学意义(P<0.05);而Runx2mRNA及蛋白的表达则显著低于S+P组及C组,差异有统计学意义(P<0.05).结论 LF-PEMF是一种无创、安全而有效的治疗早期SANFH的方法,可能机制是下调骨坏死骨组织内PPAR-γ2的表达,上调Runx2的表达,抑制脂肪生成,促进骨形成,降低骨坏死率.Objective This study was designed to study the effects of low-frequency pulsed electromagnetic fields (LF-PEMF) therapy on peroxisome proliferator-activated receptor-γ2 (PPAR-γ2) and Runt-related transcription factor 2 (Runx2) expression in steroid-induced avascular necrosis of femoral head (SANFH).Methods Forty-two Wistar rats were divided into three groups.(1) Steroid group (S,n =16),animal modeling was making by glucocorticoid and lipopolysaccharide,without exposure to LF-PEMF.(2) Steroid + LF-PEMF group (S + P,n =16),four weeks after making modeling,all rats were treated with LF-PEMF (4.5 ms square pulse,repeated at 15 Hz,with a peak of 1.2 mT) for4 h/d for the next 8 weeks.(3)Control group (C,n =10),all rats were treated without steroid use and exposure to LF-PEMF.After 8-week treatment,the histological changes,the osteonecrosis incidence,the empty osteocyte lacuna rate and the mRNA and protein expression of PPAR-γ2 and Runx2 were measured and analyzed.Results In Group S,the osteonecrosis incidence (69%) and empty osteocyte lacuna rate [(59.55 ± 21.70) %] were both higher than which in group S + P [31%,(36.16 ± 15.34) %;P < 0.05] and group C [0%,(8.79 ± 2.42)%;P < 0.05].Furthermore,in group S,the expressions of PPAR-γ2 mRNA and protein are significantly higher than that in group S + P and group C (P < 0.05),and the expressions of Runx2 mRNA and protein are significantly lower than that in group S + P and group C (P < 0.05).Conclusion LF-PEMF is an effective and safe physiotherapy in the treatment of SANFH,and the possible underlying mechanisms include increasing the expressions of Runx2 and decreasing the expressions of PPAR-γ2 in femoral head tissues,and further promoting bone formation and inhibiting adipogenesis.
