摘　　要：【目的】通过对脑卒中患者健康教育及行为危险因素的干预，探讨社区脑卒中二级预防规范方案及实施效果。【方法】选择上海市长宁区同仁医院收治的83例脑卒中患者为研究对象，对脑卒中患者及其监护人定期进行健康教育，普及疾病相关知识，并进行持续的行为危险因素的干预。一年后随访，用自行设计的《脑卒中患者干预后评判问卷》对脑卒中患者及其监护人进行问卷调查，了解脑卒中患者及其监护人对脑卒中疾病相关知识的掌握情况、不良生活习惯的改善及脑卒中复发情况。【结果】干预后，脑卒中患者及其监护人对脑卒中疾病相关知识的知晓率、脑卒中患者不良生活习惯的改善率均较干预前有显著提高，差异有统计学差异（ P ＜0．01），脑卒中复发率为4．8％。【结论】对脑卒中患者进行二级预防能显著提高其相关疾病知识的知晓率，改变其不良生活习惯，能够明显降低脑卒中的复发率。[Objective] To evaluate the outcomes of secondary prevention for cerebral stroke through health education and intervention of risk factors .[Methods] A total of 83 patients of cerebral stroke from Tongren Hospital and their guardians received regular health education and continuous intervention of risk factors .And the questionnaires were completed one year later .The awareness rate of cerebral stroke and the improvement rates of unhealthy living habits were recorded and compared with those prior to intervention and health education .And the recurrence rate of cerebral stroke was used for appraising the outcomes of secondary prevention .[Results] Finally a total of 264 questionnaires was retrieved .And statistical results showed that the awareness rate of cerebral stroke and the improvement rates of unhealthy living habits were significantly different from those prior to intervention and health education （ P 〈 0 .01） .And the recurrence rate of cerebral stroke was lower than Chinese average .[Conclusion] Secondary prevention of cerebral stroke at Changning District is successful for reducing the recurrence rate and promote the awareness rate of cerebral stroke .
