老年性非小细胞肺癌调强适形放疗同期化疗的临床研究

摘　　要：目的探讨老年性非小细胞肺癌调强适形放疗同期化疗的临床效果。方法选取2013年11月-2014年10月本院收治的80例老年性非小细胞肺癌患者,将其随机分为观察组和对照组,各40例。两组均实施顺铂联合紫杉醇治疗方案进行化疗,对照组同步实施常规放疗,观察组同步实施调强适形放疗。比较两组的临床疗效及毒副反应。结果观察组总有效率为82.50%,高于对照组的45.00%,差异有统计学意义（P〈0.05）。两组患者的药物毒性反应主要有骨髓抑制、放射性食管炎、急性放射性肺炎,两组的骨髓抑制发生率差异无统计学意义（P〉0.05）,放射性食管炎、急性放射性肺炎的发生率差异有统计学意义（P〈0.05）。结论在老年性非小细胞肺癌的治疗中,调强适形放疗同期化疗与常规放疗比较,具有效果好、毒副反应轻等特点,值得临床推广应用。Objective To investigate clinical effect of intensity modulated conformal radiation therapy concurrent chemotherapy in treatment of elderly non-small cell lung cancer. Methods 80 elderly patients with non-small cell lung cancer admitted into our hospital from November 2013 to October 2014 were selected, and they were randomly divided into observation group and the control group,there were 40 cases in each group.Cisplatin plus paclitaxel chemotherapy regimen was implemented to all patients,control group was implemented with conventional radiotherapy,while observation group was implemented with intensity modulated conformal radiation therapy.The clinical efficacy and toxic and side reaction between two groups was compared respectively. Results The total effective rate of observation group（82.50%） was higher than that of control group（45.00%）,and the difference was significant（P〈0.05）.Drug toxic and side reaction in the two groups was mainly bone marrow suppression,radiation esophagitis,acute radiation pneumonitis,the incidence rate of bone marrow suppression between two groups had no significant difference（P〉0.05）;the incidence rate of radiation esophagitis and acute radiation pneumonitis had significant difference（P〈0.05）. Conclusion In the treatment of elderly non-small cell lung cancer,compared with conventional radiotherapy,intensity modulated conformal radiation therapy concurrent chemotherapy has good efficacy with soft toxic and side reaction and other characteristics.It is worthy of clinical promotion and application.
