微型外固定支架在手部内生软骨瘤伴骨折中的应用

摘　　要：目的 探讨微型外固定支架在治疗手部内生软骨瘤伴病理性骨折中的临床疗效. 方法 2010年1月—2013年1月,对15例内生软骨瘤合并病理性骨折患者采用病灶切除加自体髂骨植骨术,并使用微型外固定支架联合克氏针固定.结果 术后切口均一期愈合.所有病例均获得随访,随访时间 12-15个月,平均13个月.手功能按照美国手外科学会的分级功能评定方法评定：优14例,良1例.结论 微型外固定支架联合克氏针在治疗手部内生软骨瘤伴病理性骨折中起到重要作用,具有固定可靠,可早期功能锻炼,明显改善手部功能等特点.Objective To introduce a surgical technique combined with mini external fixator in the treatment of enchondroma of the hand accompanied pathologic fracture. Methods From January 2010 to january 2013, 15 patients used mini external fixator and k-wires in the treatment of enchondroma of the hand accompanied pathologic fracture. All received curettement with using autologous bone grafting. The mini external fixator and K-wires internal fixation were used to stabilize the fracture. Results All got phase I healing. All cases were followed with a mean time from 12-15 months （range of 13 months）. According to the evaluation method of grading of American Society for Surgery of the Hand： 14 cases are good, 1 is general. Conclusion The surgical technique combined mini external fixator with K-wires internal fixation is a good method for the treatment of enchondroma of the hand accompanied pathologic fracture. The advantages of mini external fixator is helpful in the reduction, fixation, earlier rehabilitation, improving the function of hand.
