太极扣和磁性附着体在全口覆盖义齿中应用的5年临床效果比较

摘　　要：目的比较太极扣和磁性附着体在全口覆盖义齿中应用5年的临床效果。方法选择仅保留了个别残根残冠的牙列缺损病例,分别采用根上太极扣和磁性附着体加覆盖全口义齿修复,修复后定期复查,经过5年临床随访,对患者满意度、临床检查和X线片检查进行评价。结果患者满意度比较结果显示太极扣组和磁性附着体组在美观性、固位性、稳定性、咀嚼功能和舒适性方面均无统计学差异(P>0.05);临床检查和X线片检查结果显示太极扣组和磁性附着体组在基牙松动度、牙龈指数和牙槽骨吸收上均无统计学差异(P>0.05)。磁性附着体组有1例下颌尖牙磁铁处的人工牙在戴牙1.5年后脱落,重新修理后继续使用;太极扣组有1例下颌单基牙病例中的基牙松动度达到2度,牙槽骨吸收超过2/3;两组均有超过60%的基牙发生不同程度的牙龈炎症。结论太极扣和磁性附着体各有所长,在覆盖全口义齿修复中的5年疗效观察满意,但是要注意适应证的选择。Objective To compare 5 years＇ clinical effect of easy resilient attachment（ ERA） and magnetic attachment for complete overdenture restoration. Methods ERA and magnetic attachment were applied to patients with complete overdenture. This study had been carried out and followed- up for 5 years in order to survey the satisfaction on consequence of clinical and radiographic examinations and to evaluate the clinical effect.Results There was no significant difference observed in aesthetics,retention,stable,mastication,comfort,abutment mobility,gingival index and alveolar bone absorption between these two groups（ P〉0. 05）. In magnetic group,one artificial tooth was fell off during abutment after 1. 5 years,it had been repaired and continually used. In one case of ERA group,abutment mobility was over 2 degree,and alveolar bone absorption was over 2 /3. In these two groups,over 60% abutments were found to have gingivitis. Conclusion ERA and magnetic attachment have their own advantages,both these two groups performed well during follow- up for 5 years. But it is important that the indications should be strictly mastered.
