拉莫三嗪对癫痫患者疗效和安全性及生活质量影响研究

摘　　要：目的观察单独使用拉莫三嗪以及与其他药物联合使用拉莫三嗪治疗癫痫的临床效果。方法以某院神经内科2010年3月-2013年3月收治的120名患者作为研究对象,根据药物治疗方案的不同,将患者分为3组:第一组为拉莫三嗪治疗组;第二组为拉莫三嗪联合丙戊酸钠治疗组,第三组为丙戊酸钠治疗组。比较3组患者治疗1年后治疗总有效率、生活质量评分、不良反应发生率等指标。结果拉莫三嗪组患者治疗1年后,25例治疗有效,有效率为62.5%;拉莫三嗪联合丙戊酸钠组治疗有效者共有31人,治疗有效率为77.5%;丙戊酸钠组治疗有效者24例,治疗有效率为60.0%。拉莫三嗪联合丙戊酸钠组治疗效果与其他2组之间差异有统计学意义(P<0.05)。3组患者治疗后与治疗之前相比,生活质量均有所提高(P<0.05),丙戊酸钠组患者生活质量评分小于拉莫三嗪治疗组、拉莫三嗪联合丙戊酸钠治疗组(P<0.05)。拉莫三嗪组治疗组总不良反应率低于其余2组患者(P<0.05)。结论无论是单用拉莫三嗪还是与丙戊酸钠联合使用,拉莫三嗪均有良好的治疗效果,可以在临床上推广使用。Objective This study aimed to compare the efficacy of using lamotrigine alone and that of using it in combination with other drugs for the treatment of epilepsy. Methods A total of 120 patients admitted in the department of neurology during March, 2010 - March, 2013 were included in this study. They were divided into the lamotrigine group, the lamotrigine ＋ valproate group, and the valproate group. After one-year treatment, the three groups were compared in the total effect of treatment, indexes of life quality and incidence of adverse reactions. Results For the lamotrigine group, there were 25 cases of effective treatment, with an effective rate of 62.5%; for the lamotrigine ＋ valproate group, 31 cases of effective treatment, with an -effective rate of 77.5%; for the valproate group, 24 cases with an effective rate of 60.0%. The lamotrigine ＋ valproate treatment was significantly better than the other two groups （P〈.05）. Compared with those before treatment, the quality of life was improved in all three groups （P〉0.05）, but overall the score of life quality was valproate group 〈 lamotrigine group 〈 lamotrigine ＋ valproate group （P〈0.05）. The total adverse reaction rate of the lamotrigine group is lower than that of the rest two groups （P〈0.05）. Conclusion Lamotrigine, no matter used alone or in combination with sodium valproic acid, is effective against epilepsy, so it is recommended here for widely clinical usage.
